慢性乙型肝炎患者调节性T细胞表达水平的研究 被引量:8

摘要 目的探讨调节性T细胞(Treg)在慢性乙型肝炎患者中的作用,分析其对细胞免疫功能及肝脏炎症损伤的影响。方法 2010年7月-2012年9月采用流式细胞仪检测59例患者及20名健康对照者外周血调节性T细胞及T淋巴细胞亚群,ELISA检测叉头状/翅膀状螺旋转录因子(FOXP3)浓度,同时检测乙型肝炎病毒(HBV)血清标志物、乙型肝炎病毒载量(HBV-DNA)、肝功能,采用SPSS15.0软件对数据进行统计分析。结果与NC组比较,CHB及LC组CD4+CD25+均显著升高,差异有统计学意义(P<0.01),CHB、LC组较NC组CD4和CD4+/CD8+比值降低,CD8+增高,差异有统计学意义(P<0.05);CHB及LC患者血浆FOXP3浓度分别为(102.46±25.87)ng/L及(108.80±13.06)ng/L,均较NC组(60.77±8.45)ng/L升高,且呈水平递增,差异有统计学意义(P<0.01);随着病毒拷贝数的增加,外周血CD4+CD25+及FOXP3浓度逐渐升高,差异有统计学意义(P<0.05),CD4+CD25+与FOXP3浓度均与血清ALT水平、HBV-DNA载量呈正相关,差异有统计学意义(P<0.05)。结论调节性T细胞升高可能与HBV感染慢性化、免疫功能受损及肝脏炎症损害有关。 OBJECTIVE To investigate the role of regulatory T cells(Treg)in patients with chronic hepatitis B and analyze its impact on cellular immunological function and intrahepatic inflammation.METHODS Peripheral blood from 59 patients and 20 normal controls was analyzed for percentage of CD4^+CD25^+,CD4^+and CD8^+lymphocyte subsets by flow cytometry,and enzyme-linked immunosorbent assay(ELISA)was used to detect the level of plasma FOXP3,meanwhile,HBVM,HBV-DNA and the level of ALT for liver function were detected.RESULTS CD4^+CD25^+was significantly increased in the CHB and LC groups compared to the NC group(P〉0.01).The percentage of CD4^+and CD4^+/CD8^+was significantly decreased and CD8+was significantly increased in the CHB and LC groups compared to the NC group(P〈0.05).The level of plasma FOXP3 was 102.46±25.87ng/L in the CHB group and 108.80±13.06ng/L in the LC group,which were both significantly higher than 60.77±8.45ng/L in the NC group,and increased gradually along with the severity of disease(P〈0.01).Along with the increase of virus copy number,peripheral CD4^+CD25^+and FOXP3 concentration gradually increased,with significant difference(P〈0.05).CD4^+CD25^+and FOXP3 concentration were positively correlated with serum ALT and HBV-DNA capacity.CONCLUSION The increase of Treg may have some relationship with the development of chronicity of HBV infection,immune function impairment and intrahepatic inflammation.
