HBV—LP阳性患者CD4^+CD25^+Treg细胞表达的研究

摘要 目的探讨HBV—LP(乙肝病毒大蛋白)阳性患者CD4^+CD25^+Treg细胞数量的变化规律及其意义.方法将HBV感染者标本,采用ELISA方法检测HBV—LP,PCR方法检潮HBV—DNA,按照HBV-LP阴、阳性患者反正常血清共170例,其中男87例,女83例,平均年龄43(23~61)岁。分成四个实验组,第一组为HBV—LP阳性46例,第二组为正常组(非HBV感染者)50例,第三组为HBV—LP阴性及HBV—DNA阴性HBV意来者43例,第四组为HBV—LP阴性而HBV—DNA阳性的HBV感染者31例。各组之间在性别、年龄上分布均衡,差异无统计学意义。各组分别采用淋巴细胞分离液梯度离心法分离研究对象外周血,FCM术检测CD3^+T,CD4^+T,CD8^+T,CD4^+CD25^+Treg细胞数量,进行分析.结果①各实验组之同CD3^+T,CD8^+T细胞数量无显著性差异,各组CD3^+T分别为(65.38±4.85)%,(67.07±5.04)%,(64.94±5.26)%,(63.58±4.81)%(P〉0.05),CD8^+T分别为(53.41±4.28)%,(54.71±5.05)%,(54.09±5.13)%,(52.59±4.88)%(P〉0.05)。②结果以CD4^+CD25^+Treg来评价:HBV—LP阳性组(5.22±1.81)%分别高于正常对照组(3.13±1.12)%,HBV—LP阴性及HBV—DNA阴性组(3.24±1.22)%,HBV—LP阴性而HBV—DNA阳性组(3.23±1.61)%(P〈0.01),而HBV-LP阳性组CD4细胞数量(32.63±3.93)%却低于其他三个实验组(39.32±5.55%),(37.53±4.72)%,(34.03±5.71)%,差异有统计学意义(P〈O.05),③值得注意的是,HBV—DNA阳性而HBV—LP阴性组的各项指标几乎与正常组十分接近,之间差异无统计学意义(P〉0.05)。结论HBV—LP阳性的HBV感染者外周血CD4^+CD25^+Treg细胞水平明显增高,CD4^+CD25^+Treg细胞的过度表达在乙肝的发病机制及病程的转归中可能起着重要的作用。 Objective To explore HBV-LP (hepatitis B virus large surface protein)-positive patients with CD4^+CD25^+Treg cells and its significance changes. Methods HBV infected samples,using ELISA,HBV-LP,PCR to detect HBV-DNA,in accordance with the HBV-LP negative,the positive patients and normal serum were 170 cases, 87 patients were male,83 female, mean age 43 (23-61) years. Divided into four experimental groups,the first group,46 patients positive for HBV-LP, the second group of the normal group (non-HBV infected) of 50 cases,the third group,HBV-LP-negative and HBV-DNA negative in 43 cases of HBV infection the fourth group,HBV-LP-negative and HBV-DNA-positive HBV infection in 31 cases. Among the groups in gender ,age distribution of balance ,the difference was not statistically significant. Each group was used to lymphocyte isolation gradient centrifugation of peripheral blood study,FCM cytometry CD3^+T,CD4^+T,CD8^+T and CD4^+CD25^+Treg cells for analysis. Results (1)Experimental groups CD3+T,CDS+T cells no significant difference in CD3^+T groups were(65. 38±4.85)%, (67. 07±5.04)%, (64. 94±5.26)% and (63. 58±4.81)% (P〉 0. 05),CD8^+T were (53. 41±4. 28)%, (54. 71±5. 05)%, (54. 09±5.13)% and (52. 59±4. 88)%(P〉0. 05). (2)Results by CD4^+CD25^+ Treg to evaluate :HBV-LP-positive group (5. 22±1.81)% were higher than the control group (3. 13±1.12)% ,HBV-LP- negative and HBV-DNA negative group (3.24 ±1.22)%, HBV -LP-negative and HBV-DNA positive group (3. 23 ± 1.61) %, (P〈0. 01) ,and HBV-LP-positive group CD4 cell count (32. 63±3.93)% was lower than the other three experimental groups (39. 32±5.55)%, (37. 53±4.72)%, (34. 03±5. 71)% ,the difference was significant (P〈0. 05),(3)Note the that HBV-DNA positive and HBV-LP-negative group,the index was almost very close to the normal group (P〉 0.05).
