基于体素的MR形态学分析对鼻咽癌放疗后脑灰质体积改变的研究 被引量:5

Voxel-based Morphometry MRI Study of Gray Matter Volume Changes in Patients with Nasopharyngeal Carcinoma Following Radiation Therapy

摘要 目的运用基于体素的形态学分析(VBM)技术探讨鼻咽癌患者放疗后全脑灰质体积的改变情况。方法纳入初诊放疗前(G0组)、放疗后0-6月(G1组)、6-12月(G2组)的鼻咽癌患者各15例,采用3.0T MR扫描仪,进行高分辨力T1WI扫描,采用基于SPM8软件的VBM8工具箱分别分析治疗组(G1组、G2组)与对照组(G0组)脑灰质体积变化的差异。结果与G0组比较,G1组双侧额叶(右侧中央前回、双侧额上回、双侧额中回),左侧后扣带回、左侧岛叶灰质体积减少。而对比G0组,G2组双侧颞叶(左侧颞叶钩回、右侧梭状回)、左侧额叶(左侧额下回、左侧额中回)灰质体积减少;右侧扣带回及左侧顶下小叶出现灰质容积的增加。结论鼻咽癌放疗后不同时期与放疗前存在脑灰质体积的异常,而VBM技术可以敏感地检测这种变化。 Objective To investigate the changes of gray matter volume in nasopharyngeal carcinoma (NPC) patients after radiotherapy by voxel-based morphometry analysis. Methods Forty-five NPC patients were prospectively equally assigned into 3 groups in this study: NPC before radiotherapy (G0) versus NPC after radiotherapy for 1ess than 6 months (G1) or for 6-12 months (G2). All cases underwent a high-resolution T1WI scanning using a 3.0 T MR scanner. The volume changes of gray matter were analyzed using SPM8 and VBM8 toolbox. Results Compared to G0, the area of gray matter volume reduction in G1 are:bilateral frontal gyrus (right precentral gyrus, bilateral superior frontal gyrus, and bilateral middle frontal gyrus), left posterior cingulated, left insula. Whereas compared to G0, G2 exhibited reduced gray matter volume in bilateral temporal lobes (left uncus, right fusiform gyrus), left frontal gyrus (left inferior frontal gyrus, left middle frontal gyrus); and increased gray matter volume in right cingulated gyrus, left inferior lobule. Conclusion The volume of gray matter changed after radiotherapy in NPC patients, and VBM technique can sensitively detect the differences.
