摘要 目的 观察以时辰给药法行新辅助化疗与常规新辅助化疗治疗Ⅱb、Ⅲ期乳腺癌的近期疗效、毒副作用。方法 时辰化疗组用药为顺铂 (DDP) ,给药时间为 4pm ;氟脲嘧啶 ( 5 FU)给药时间为 10pm~ 10am ,在 3am~ 5am给予 5 FU药物总量的 75 %。常规新辅助化疗组所用药物及剂量同时辰化疗组 ,用药时间安排在正常上班时间 ( 8am~ 5pm)。完成1周期及以上化疗者 ,评价化疗近期毒副反应 ,完成 2周期及以上化疗者评价化疗近期疗效 ;化疗结束后 1月行手术治疗 ,据手术后病理评价化疗的病理完全缓解率。结果 时辰化疗组的临床完全缓解率 (CR) 38.98%VS 2 3.5 %、有效率(RR) 83.7%VS70 .6 %、病理完全缓解率 2 6 .5 %VS13.2 %均显著高于常规化疗组 (P <0 .0 5 ) ,时辰化疗组Ⅲ、Ⅳ度白细胞下降显著低于常规化疗组 (P <0 .0 5 ) ,减量化疗周期数、延期化疗周期数及其它近期化疗毒副反应两组间均无显著差异 (P>0 .0 5 )。结论 在乳腺癌的新辅助化疗中用FP方案化疗时 ,时辰化疗组的近期疗效显著高于常规化疗组 ,二组的化疗近期毒副反应基本相同 ,在乳腺癌的新辅助化疗中可采用时辰化疗法。 Objective To observe the short term efficacy and toxic side effects of the neoadjuvant chemotherapy with cisplatin and fluorouracil in stage Ⅱb or Ⅲ breast cancer by a chronomodulated in fusion method and a routine infusion method respectively.Methods In order to coinide with relevant circadin rhythms,cisplatin was administered at 4 p.m., with fluorouracil from 10 p.m. to 10 a.m. of which 75% was given between 3 am and 5 am in Group A.In group B the same doses of cisplatin and fluorouracil were given in the daily working time from 8 am to 5 pm by the routine infusion method.The short term toxic side effects in those who underwent more than one cycles and the short term efficacy in those who completed more than two cycles were evaluated respectively.One month after the neoadjuvent chomotherapy,surgery was carried out.After operation the complete response was also evaluated.Results Complete response rate and response rate were significantly higher in Group A than those in group B(P<0.05).Grade 3 or 4 toxic side effects were mainly the decrease of white blood corpuscles which were remarkably lower in Group A than in Group B(P<0.05).There was no significant difference in the number of dose-reduction cycels,delay cycles as well as other short term toxic side effects between these two groups(P>0.05).Conclusion The short term efficacy of FP regimen in the neoadjuvant chemotherapy of breast cancer is apparently higher in the chronotherapy group(A) than that in the routine group(B).However,there is no difference in the short term toxic side effects between these two groups.The chronomdulated infusion method is useful in the neoadjuvent chemotherapy of breast cancer.
