摘要
目的探讨基于FOCUS-PDCA程序的护理干预对脑卒中伴抑郁症患者抑郁焦虑情绪,睡眠障碍及生活质量的影响。方法选择我院2015年10月～2016年10月收治的脑卒中伴抑郁症患者98例为研究对象,随机将其分为对照组与观察组,各49例。对照组采用常规护理,观察组在常规护理基础上采用基于FOCUS-PDCA程序的护理干预,比较两组患者的不良情绪、睡眠障碍情况及生活质量。结果干预后两组患者医院焦虑抑郁调查量表(HADS)评分均较干预前有所下降(P<0.05),且观察组HADS评分明显低于对照组(P<0.05);干预后两组患者匹兹堡睡眠质量指数(PSQI)评分均低于干预前(P<0.05),且观察组患者睡眠效率、睡眠时间、睡眠质量、日间功能、睡眠障碍及安眠药物评分明显低于对照组(P<0.05);干预后两组生活质量综合评定问卷(GQOLI-74)躯体功能、社会功能、心理功能及物质生活状态评分均有所提高(P<0.05),且观察组患者各维度评分明显高于对照组(P<0.05)。结论基于FOCUS-PDCA程序的护理干预对减轻脑卒中伴抑郁症患者抑郁、焦虑等不良情绪效果显著,在改善患者睡眠障碍、提高生活质量方面均具有重要意义。
Objective To explore the influence of nursing intervention based on FOCUS-PDCA-program on anxiety and depression mood,sleep disorder and quality of life in stroke patients with depression.Methods 98 cases of stroke patients with depression treated in our hospital from October 2015 to October 2016 were selected as the study objects,and were divided into control group and observation randomly,49 cases in each group.The control group were given routine nursing,and the observation group were given nursing intervention based on FOCUS-PDCA program on the basis of routine nursing.Unhealthy mood,sleep disorder and quality of life were observed in the two groups. Results After intervention,the anxiety and Depression Scale (HADS)scores of the two groups were decreased (P < 0.05),and the HADS score of the observation group was significantly lower than that of the control group (P <0.05). The sleep of patients in the two groups after the intervention of Pittsburgh quality index (PSQI )scores were lower than before intervention (P<0.05),and the patients in the observation group sleep efficiency,sleep time,sleep quality, daytime dysfunction,sleep disorders and sleeping pills was significantly lower than the control group (P <0.05).After the intervention,two groups of quality of life assessment questionnaire (GQOLI-74)and physical function,social function,psychological function and living status score were improved (P<0.05),and the observation group with each dimension score significantly higher than the control group (P<0.05).Conclusion The effect is obvious of nursing intervention based on FOCUS-PDCA program in reducing the level of depression and anxiety mood of stroke patients with depression.It is of great significance to improve the sleep disorder and quality of life.
