血糖波动与糖尿病肾病患者机体炎症及肝肾功能的关系 被引量:25

摘要 目的探讨血糖波动与糖尿病肾病(DN)患者机体炎症及肝肾功能的关系。方法将90例DN患者根据平均血糖波动幅度(MAGE)情况分为稳定组(n=49)和波动组(n=41)。检测两组患者血超敏C反应蛋白(HsCRP)、肿瘤坏死因子-α(TNF-α)、白介素-6(IL-6)等炎症指标及肝肾功能指标水平,分析各指标与患者MAGE的相关性。结果波动组患者血清Hs-CRP、TNF-α及IL-6等炎症指标水平均高于稳定组(P<0.05)。波动组患者血丙氨酸转氨酶(ALT)、天冬氨酸转氨酶(AST)、总胆红素(TBIL)等肝功能指标及胱抑素C(CysC)、肌酐(Scr)、尿素氮(BUN)等肾功能指标水平也高于稳定组(P<0.05)。DN患者血清Hs-CRP、TNF-α、IL-6及肝肾功能指标水平与MAGE均呈显著正相关(P<0.05)。结论血糖波动幅度过大会加剧DN患者机体炎症状态,损害患者肝肾功能。 Objeetive To investigate the relationship between glycemic excursion and inflammation/hepatorenal function in patients with diabetic nephropathy (DN).Methods 90patients with DN were divided into stable group (n =49)and wave group (n=41)according mean amplitude of glyeemic excursions (MAGE).The inflammation indexes of high-sensitivity C-reactive protein (Hs-CRP),tumor necrosis factor-α (TNF-α),interleukin-6(IL-6)and hepatorenal function indexes were detected.The relationships between those indexes and MAGE were analyzed.Results The levels of serum Hs-CRP,TNF-a and IL-6in wave group were significantly higher than stable group (P<0.05).The liver function indexes of alanine aminotransferase (ALT),aspartate transaminase (AST),total bilirubin (TBIL)and renal function indexes of cystatin C (CysC),serum creatinine (Set),blood urea nitrogen (BUN)in wave group were signifi- cantly higher than stable group (P<0.05).The levels of serum Hs-CRP,TNF-a,IL-6and hepatorenal function index in DN patients were significantly positively correlated with MAGE (P<0.05).Conclusion The dramatic change of blood glucose can increases the inflammatory state of DN patients and impaired the liver and kidney function.
