多学科营养护理干预在急性胰腺炎患者中的应用 被引量:5

摘要 目的探讨多学科营养护理干预在急性胰腺炎患者中的应用价值。方法 100例急性胰腺炎患者,随机分为观察组与对照组,各50例。对照组给予常规护理,观察组在对照组基础上予以多学科营养护理干预,比较记录两组患者腹痛缓解时间、住院时间、淀粉酶复常时间,干预前后总蛋白、血红蛋白、p H指标水平,急性生理与慢性健康评分系统Ⅱ(APACHEⅡ)、焦虑自评量表(SAS)评分、简明健康情况调查表(SF-36)评分及并发症发生情况。结果观察组腹痛缓解时间、住院时间、淀粉酶复常时间分别为(2.35±0.29)、(22.15±1.87)、(4.85±0.56)d,均明显短于对照组的(5.56±0.75)、(31.02±0.47)、(7.54±1.20)d,差异具有统计学意义(P<0.05)。两组患者干预前总蛋白、血红蛋白、p H指标水平比较,差异无统计学意义(P>0.05)。两组患者干预后总蛋白、血红蛋白、p H指标水平较干预前显著升高,且观察组高于对照组,差异均具有统计学意义(P<0.05)。干预前两组急性生理与慢性健康评分(APACHEⅡ)、SAS评分、SF-36评分比较差异无统计学意义(P>0.05)。干预后,两组APACHEⅡ、SAS评分较干预前显著降低、SF-36评分较干预前显著升高,且观察组APACHEⅡ、SAS评分明显低于对照组, SF-36评分明显高于对照组,差异均具有统计学意义(P<0.05)。观察组干预后并发症发生率8.00%低于对照组的24.00%,差异具有统计学意义(P<0.05)。结论多学科营养护理干预应用于急性胰腺炎患者中,可明显改善其营养指标及血清学指标,纠正酸碱平衡,缩短住院时间,减少并发症的发生,值得在临床推广应用。 Objective To discuss the application value of multidisciplinary nutritional nursing intervention in patients with acute pancreatitis. Methods A total of 100 patients with acute pancreatitis were randomly divided into observation group and control group, with 50 cases in each group. The control group received conventional nursing, and the observation group received multidisciplinary nutritional nursing intervention on the basis of the control group. Comparison and record were made on abdominal pain remission time, hospitalization time, amylase recovery time, total protein, hemoglobin and pH index before and after intervention, acute physiology and chronic health evaluation (APACHE) Ⅱ score, self-rating anxiety scale (SAS) score, 36-item short-form (SF-36) score and complications between the two groups. Results The observation group had obviously shorter abdominal pain remission time, hospitalization time and amylase recovery time respectively as (2.35±0.29), (22.15±1.87) and (4.85±0.56) d, than (5.56±0.75), (31.02±0.47) and (7.54±1.20) d in the control group, and their difference was statistically significant (P<0.05). There was no statistically significant difference in total protein, hemoglobin and pH before intervention (P>0.05). After intervention, both groups had significantly higher total protein, hemoglobin and pH than those before intervention, and the observation group was higher than the control group. Their difference was statistically significant (P<0.05). Before intervention, there was no statistically significant difference in APACHE Ⅱ , SAS score and SF-36 score (P>0.05). After intervention, both groups had significantly lower APACHE Ⅱ and SAS score and higher SF-36 score than those before intervention. The observation group had obviously lower APACHE Ⅱ and SAS score, and obviously higher SF-36 score than the control group. Their difference was statistically significant (P<0.05).
