摘要
目的:观察桂枝茯苓丸加味联合米非司酮治疗寒凝血瘀型子宫肌瘤的疗效及对患者血清炎症因子的影响。方法:将116例患者按随机数字表法分为2组,对照组58例采用米非司酮治疗,观察组58例在对照组基础上加服枝茯苓丸加味治疗;疗程均为3月,观察2组患者治疗前后经期症状及临床症状变化情况,检测其血清肿瘤坏死因子-α(TNF-α)、白细胞介素-6 (IL-6)、白细胞介素-8 (IL-8)、促黄体生成素(LH)、孕酮(P)、卵泡生成素(FSH)、雌二醇(E_2)含量。结果:治疗后2组患者经前乳胀、月经量多、腰酸、小腹痛及血块等经期症状发生率均较治疗前降低(P <0.05);且观察组上述各症状发生率低于对照组(P <0.05)。治疗后2组患者手心热、肛门痛、性急易怒、性交痛、尿频、口干、乏力等临床症状发生率均较治疗前降低(P <0.05);且观察组患者性急易怒、口干、乏力等症状发生率低于较对照组(P <0.05)。治疗后2组患者血清TNF-α、IL-6、IL-8、LH、P、FSH、E_2水平均较治疗前降低(P <0.05);且观察组上述指标改善较对照组更显著(P <0.05)。结论:桂枝茯苓丸加味联合米非司酮治疗可显著改善寒凝血瘀型子宫肌瘤患者经期症状及临床症状,改善血清炎症因子及性激素水平,疗效优于单纯西药治疗。
Objective:To observe the clinical effect of modified Guizhi Fuling pills combined with mifepristone for hysteromyoma of blood stasis due to congealing cold type,and its effect on inflammatory factors in serum of patients. Methods:Divided 116 cases of patients into two groups randomly, 58 cases in each group. The control group received mifepristone,while the observation group additionally received the oral administration of modified Guizhi Fuling pills. The course of treatment lasted for 3 months. Observed the changes of menstrual symptoms and clinical symptoms of patients in both groups before and after treatment,detected the contents of tumor necrosis factor-α(TNF-α), interleukin-6 (IL-6), interleukin-8 (IL-8), luteinizing hormone (LH), progesterone (P), follicle stimulating hormone (FSH) and estradiol (E2) in serum. Results: After treatment, the incidence of such menstrual symptoms as premenstrual mammary swelling pain,menorrhagia,waist soreness,lower abdomen pain and blood clot in both groups was respectively lower than that before treatment (P<0.05); the incidence of each symptom above in the observation group was decreased more significantly than that in the control group (P<0.05). After treatment, the incidence of such clinical symptoms as feverish palms, proctalgia, impetuosity and irascibility, intercourse pain, pollakiuria, dry mouth and hypodynamia in both groups was respectively lower than that before treatment(P<0.05); the incidence of such clinical symptoms as impetuosity and irascibility, dry mouth and hypodynamia in the observation group was decreased more significantly than that in the control group(P<0.05). After treatment,the levels of TNF-α,IL-6,IL-8,LH,P,FSH and E2 in serum of patients in both groups were lower than those before treatment (P < 0.05);each index above in the observation group was improved more significantly than those in the control group(P<0.05).
