摘要
目的应用基质辅助激光解吸电离飞行时间质谱(MALDI TOF MS)及VITEK 2Compact全自动微生物鉴定仪对血培养厌氧菌的鉴定,提高对兼性厌氧菌及专性厌氧菌的检出水平。方法分析医院2018年5月14日入住ICU 1例颊普雷沃菌和极小陌生菌所致的右侧颌面部间隙感染病例,观察血培养厌氧瓶阳性报警曲线,接种厌氧血琼脂厌氧环境培养,观察菌落形态,涂片革兰染色。采用MALDI TOF MS及VITEK 2Compact进行鉴定。结果血培养(需氧瓶和厌氧瓶)厌氧培养鉴定为颊普雷沃菌和极小陌生菌,穿刺液注入一套血培养(需养瓶和厌氧瓶)厌氧培养鉴定为颊普雷沃菌和极小陌生菌,VITEK 2Compact全自动微生物鉴定仪鉴定结果为颊普雷沃菌,符合率99%;基质辅助激光解吸电离飞行时间质谱(MALDI TOF MS)鉴定结果分别显示为颊普雷沃菌,分值2.009,极小陌生菌,分值2.277。结论结合临床病例厌氧菌感染多为混合感染,且培养、鉴定要求条件较高。加强对厌氧菌感染的认识和实验室检测,为临床提供准确的病原学依据,进行有针对性的治疗。
OBEJCTIVE To identify anaerobic bacteria in blood culture by matrix-assisted laser desorption ionization time-of-flight mass spectrometry(MALDI TOF MS)and VITEK 2 Compact automatic microbial identification instrument,and to improve the detection level of facultative anaerobic bacteria and obligate anaerobic bacteria.METHODS One case of maxillofacial space infection caused by Prevotella buccae and Atopobium parvulumadmitted to the hospital on May 14 2018 was analyzed.The positive alarm curve of blood culture in anaerobic bottle was observed,and the bacteria were inoculated to an anaerobic blood agar and cultured in an anaerobic environment.The morphology of the colonies was observed and analyzed with gram staining.Identification of the bacteria were conducted with MALDI TOF MS and VITEK 2 Compact automatic microbial identification instrument.RESULTS Anaerobic blood culture in aerobic bottle and anaerobic bottle identified the bacteria as P buccae and A parvulum.Anaerobic blood culture of punctate in aerobic bottle and anaerobic bottle identified the bacteria as P buccae and A parvulum.The VITEK 2 Compact automatic microbial identification instrument identified the bacteria as P buccae,and the identification rate was 99%.The results of MALDI TOF MS suggested the bacteria as P buccae with a score of 2.009 and A parvulum with a score of 2.277.CONCLUSION The anaerobic bloodstream infections in clinical cases are dominated by mixed infection,which has a high demand on culture and identification conditions.It was important to increase the detection rates of anaerobic bacteria and provide the etiological basis for clinical treatment,in order to reduce the incidence and mortality of anaerobic bloodstream infections.
