摘要
目的:观察活血止痛汤联合不同手术切口切开复位钢板内固定术对气滞血瘀型胫腓骨中下段双骨折患者临床疗效。方法:选取2013年5月~2017年5月间在本院接受治疗的气滞血瘀型胫腓骨中下段骨折患者76例,依据随机数字表法将患者分为两组:观察组(38例)采用小腿外侧单切口治疗,对照组(38例)常规双切口治疗;术后将两组患者再次随机分组,观察1组(19例)、观察2组(19例)、对照1组(19例)及对照2组(19例),观察1组和对照1组术后行常规护理,观察2组和对照2组在常规护理基础上服用活血止痛汤;观察患者手术、术后恢复及并发症情况,骨折愈合Johner-Wruh评分、疼痛视觉模拟评分(VAS)及肿胀恢复情况。结果:观察组患者骨痂的出现时间、手术时间、切口的愈合时间、出血量及并发症均低于对照组,对比差异有统计学意义(P <0. 05);观察组患者骨折愈合的Johner-Wruh评分优良率为92. 11%,高于对照组的68. 42%,对比差异有统计学意义(P <0. 05);对照2组、观察2组VAS评分及肿胀缓解状况均优于对照1组、观察组1组,对比差异均有统计学意义(P <0. 05)。结论:小腿外侧单切口复位钢板内固定手术比常规双切口手术的手术时间及相关并发症更少;骨折前期服用活血止痛汤可明显缓解患者肿痛情况。
Objective: To analyze the clinical effect of Huoxue Zhitong Decoction combined with open reduction and internal fixation with different surgical incision on double fracture of the middle and lower tibia and fibula with Qi stagnation and blood stasis type. Methods: From May 2013 to May 2017 between the tibia and fibula in our hospital for treatment of fractures in patients with 76 cases,according to the random number table method,the patients were divided into two groups: the observation group( 38 cases) used ectocnemial single incision,the controlled group( 38 cases) used conventional double incision; The two groups of patients again grouped randomly after operation,observation 1 groups( 19 cases) and observation group 2( 19 cases) and the controlled group 1( 19 cases) and the controlled group 2( 19 cases),the observation group 1 and the controlled group 1 patients received routine nursing,the observation group 2 and the controlled group 2 took Huoxue Zhitong Decoction on the basis of conventional care; Recovery and complications were observed in patients with surgery and postoperative healing,Johner-Wruh score,visual analogue scale( VAS) and swelling fracture recovery. Results: Patients with callus emergence time,operation time,incision healing time,complications and the amount of bleeding was lower than the controlled group,there was statistically significant difference( P < 0. 05); Fracture healing of the patients in the observation group of Johner-Wruh score excellent rate was 92.11%,68. 42% higher than that of the controlled group,there was statistically significant difference( P < 0. 05); The controlled group 2,the observation 2 group of VAS score and the swelling remission status was better than the controlled group 1,the observation 1 group,compared the differences were statistically significant( P < 0. 05).
