磁共振功能定量参数与乳腺癌生物预后因子的相关性研究 被引量:9

The Correlation Between the Quantitative Parameters of MRI and Biological Factors in Breast Cancer

摘要 目的探讨磁共振功能定量参数与乳腺癌生物预后因子表达之间相关性,旨在从影像学角度预测病灶生物预后因子表达状况,为个体化治疗方案的选择及预后预测提供一定依据。方法搜集经病理证实的34例女性乳腺癌患者(36个病灶)纳入本研究。34例患者术前均行3.0T磁共振常规扫描、扩散加权成像(DWI)及扩散峰度成像(DKI),所获MRI图像清晰,可供定量分析,均有完整免疫组织化学检查信息,包括雌激素受体(ER)、孕激素受体(PR)、人表皮生长因子受体-2(CerbB-2)和细胞增殖抗原标记物(Ki-67)。通过micron软件测量获得乳腺癌病灶的表观扩散系数(ADC)、平均扩散系数(MD)和平均扩散峰度(MK)值。根据生物预后因子表达状态将病例分成阳性组和阴性组,采用t检验和Mann-WhitneyU秩和检验,比较两组之间各定量参数间的差异。采用Spearman等级相关分析,分析各定量参数与生物预后因子表达间的相关性。结果CerbB-2阳性组的ADC、MK值高于阴性组(P<0.05),Ki-67阳性组的MK值高于阴性组(P<0.05),其差异均有统计学意义。ADC、MK值与CerbB-2表达呈正相关(r=0.339,P<0.05;r=0.522,P<0.05);MD值与Ki-67表达呈负相关(r=-0.511,P<0.05);MK值与Ki-67表达呈正相关(r=0.566,P<0.05)。结论磁共振功能定量参数ADC、MD、MK值与乳腺癌生物因子CerbB-2、Ki-67的表达具有一定的相关性。因此磁共振功能定量参数可以从影像学角度预测病灶生物因子表达状况,为个体化治疗方案的选择及预后预测提供一定依据。 Objective To analyze the correlation between the quantitative parameters of magnetic resonance imaging( MRI) and prognostic factors of breast cancer,to find MRI features which can predicted the molecular phenotypic expres-sion. Methods 34 patients with 36 lesions confirmed by pathology were collected. All patients underwent routine MRscan,DWI and DKI scan. The apparent diffusion coefficient( ADC) ,mean diffusivity( MD) and mean kurtosis( MK) valueswere measured by micron software. The breast cancer biological factors ( ER,PR ,CerbB-2 and Ki-67) were obtained byimmunohistochemistry. According to the expression status of ER、PR、CerbB-2、Ki-67,all these cases were divided into posi-tive and negative groups,the independent sample t-test and Mann-Whitney U test were used to compare the parameters be-tween two groups. The correlations between quantitative parameters ( ADC、MD、MK) and expression of biological factorswere performed using Spearman rank correlation analysis. Results ADC and MK values were higher in breast cancer withCerbB2 expression in positive than in negative( P <0. 05) ,and MK value was also higher in breast cancer with Ki-67 ex-pression in positive than in negative( P <0. 05) . The expression of ADC and MK values were positively correlated with theexpression of CerbB-2 in breast cancer . MD value of breast cancer and expression of Ki-67 was negatively correlated. MKvalue was positively correlated with Ki-67 in breast cancer. Conclusion There were correlations between MRI parametersand biological factors of breast cancer,which may be helpful for choosing treatment regimen and evaluating the prognosis.
