Correlation Between Apparent Diffusion Coefficient Value on Diffusion Weighted Imaging and Molecular Subtypes of Breast Invasive Ductal Carcinomas

摘要 目的探讨表观扩散系数(ADC)值与乳腺浸润性导管癌4种分子亚型(Luminal A、Luminal B、HER2过表达、三阴性乳腺癌)及其生物学预后因子雌激素受体(ER)、孕激素受体(PR)、人类表皮生长因子受体-2(HER2)的相关性,为鉴别不同分子亚型及评估乳腺癌预后提供理论依据。资料与方法回顾性分析具有完整病理及术前MRI资料的88例乳腺浸润性导管癌患者,术前采用平面回波成像序列获得扩散加权成像(b=0、1000 s/mm2),测定最小ADC值;术后采用免疫组化检查获得检测ER、PR、HER2及4种分子亚型,其中HER2(++)或(++^+++)采用免疫荧光原位杂交法检测,确定有无扩增。比较不同分子亚型间及乳腺癌生物学预后因子ER、PR、HER2的表达。结果乳腺浸润性导管癌4种分子亚型间ADC值差异无统计学意义(P>0.05);ER、PR阴性者ADC值高于ER、PR阳性者,差异有统计学意义(P<0.05),HER2阳性、阴性者ADC值差异无统计学意义(P>0.05)。结论 ADC值不能鉴别乳腺浸润性导管癌4种分子亚型,无法评估其生物学预后。 Purpose To investigate the correlation between apparent diffusion coefficient(ADC) value and the four molecular subtypes of breast invasive ductal carcinomas(Luminal A, Luminal B, HER-2 over-expressed), and its biological prognostic factors estrogen receptor(ER), progesterone receptor(PR), human epidermal growth factor 2,(HER2) to provide the theory basis for identifying the different molecular subtypes and evaluating prognosis of breast cancer. Materials and Methods Eighty-eight patients diagnosed as breast invasive ductal carcinomas with complete pathologic and preoperative MR data were retrospectively analyzed, preoperative diffusion weighted imaging were acquired using echo planar imaging sequence(b=0 and 1000 s/mm2), the minimum ADC was measured; ER, PR and HER2 were obtained by immunohistochemistry, and the subtype of each case was decided, HER2 was measured in fluorescence in situ hybridization to determine the presence of amplification. The expression status of the four molecular subtypes and the biological prognostic factors of ER, PR and HER2 were compared. Results The differences of ADC values among the four molecular subtypes of breast invasive ductal carcinomas were not statistically significant(P〈0.05); ADC value of ER-negative and PRnegative group was higher than that in ER-positive and PR-positive group, the difference was statistically significant(P〈0.05), while there was no significant difference between HER2 negative and positive groups(P〈0.05). Conclusion ADC value could neither be used to identify the molecular subtype of breast invasive ductal carcinomas, nor to evaluate their biological prognosis.
