摘要
目的探讨医师诊治慢性前列腺炎(CP)的潜在影响因素。方法对来自全国29个省市的泌尿外科医师进行问卷调查,使用单因素和多因素logistic回归分析模型对数据进行统计学分析以确定影响因素。结果共收回问卷1 087份,其中有效问卷1 025份。多因素logistic回归分析显示:是否采用两杯法或四杯法、学历是影响医师使用National Institutes of Health(NIH)分类法与否的关键因素。是否认为细菌感染是CP的病因是影响医师是否常规行细菌培养及是否使用抗生素的关键因素。是否认为心理因素是CP的病因是影响医师行心理评估与否的因素。是否认为心理因素是CP的病因、是否行心理评估、工作年限为影响医师是否使用心理治疗的关键因素。是否认为膀胱和盆底功能障碍是CP的病因为影响医师测定尿流率与否的关键因素。是否认为膀胱和盆底功能障碍是CP的病因、工作年限为影响医师是否使用α-受体阻滞剂的关键因素。结论医师个人对疾病的认知及诊治观念是CP诊断与治疗的重要影响因素。
Objective To explore the potential influence factors for the diagnosis and treatment of chronic prostatitis (CP). Methods A questionnaire survey was carried out in the urologists from 29 provinces of China. Univariate and multivariate logistic regression analysis were used to analyze the data and determine the influence factors. Results 1087 questionnaires were collected, of which 1025 were valid questionnaires. Multivariate logistic regression analysis showed: Whether or not to adopt the 2- or 4-glass test and education background were key factors to determine the use of National Institutes of Health (NIH) classification system. The concept of bacterial infection-caused CP was the key factor for implementation of routine bacterial culture test and the use of antibiotics. The concept of psychological factors-caused CP was a factor to decide the implementation of psychological assessment. The concept of psychological factors-caused CP, the implementation of psychological assessment and working seniority were crucial factors to determine the use of psychotherapy. The concept of bladder and pelvic floor dysfunction-caused CP was a factor to influencing the implementation of urinary flow rate detection. Concept of bladder and pelvic floor dysfunction-caused CP and working seniority were crucial factors to affect the use of alpha-receptor blockers. Conclusion The urologists' cognition of CP and individual concept were important factors for improving diagnosis and treatment of CP.
