摘要
目的比较腹腔镜阑尾切除术与小切口阑尾切除术治疗合并2型糖尿病的老年急性阑尾炎患者的临床疗效。方法纳入2014年5月至2017年12月本院普外科收治的113例合并2型糖尿病的老年急性阑尾炎患者,根据手术方式的不同分为腹腔镜组(72例)与小切口组(41例),回顾性分析比较两组的手术时间、术中出血量、术后首次肛门排气时间、并发症发生率、住院时间、住院总费用等指标。结果两组患者一般资料、术前相关情况、术中出血量等比较差异无显著性(P>0.05)。与小切口组比较,腹腔镜组手术时间短、术后并发症少、住院时间短,但住院总费用较高,差异均有显著性(P<0.05)。结论腹腔镜手术治疗合并2型糖尿病的老年急性阑尾炎具有创伤小、术后恢复快、并发症少及住院时间短等优势。
Objective To investigate the efficacy of laparoscopic appendectomy with small incision appendectomy in the treatment of elderly patients with acute appendicitis combined with type 2. Method Collected 313 cases of patients with clinical data who met the research conditions in May 2014-Dec 2017. The patients were divided into laparoscopy group(72 cases) and mini-incision n group(41 cases) according to different surgical procedures, retrospectively analyzed the operation time, intraoperative bleeding, first anal exhaust time, complication rate, length of hospitalization, total hospitalization cost, etc. Result There was no statistical difference between the two groups in general information, preoperative correlation and intraoperative blood loss(P>0.05). Compared with the mini-incision group, the operation time of the laparoscope group was shorter, the postoperative complications were less and the length of hospital stay was shorter, but the total cost of hospitalization was more, and the difference was statistically significant(P<0.05). Conclusion Laparoscopic surgery for the treatment of elderly appendicitis with type 2 diabetes mellitus has the advantages of little injury, rapid postoperative recovery, less complications and shorter hospital stay, which is an ideal procedure.
