摘要
目的分析经脐单孔腹腔镜阑尾切除术治疗急性阑尾炎的临床效果。方法选择2017年1月至2019年1月因急性阑尾炎在南京大学医学院附属泰康仙林鼓楼医院接受手术治疗的患者96例,随机分为观察组与对照组,每组48例。对照组采用传统开腹手术治疗,观察组采用经脐单孔腹腔镜阑尾切除术治疗。比较两组患者的手术情况、疼痛程度及并发症发生情况。结果观察组手术时间明显高于对照组,术中出血量、下床活动时间、肛门排气时间、术后进食时间、胃肠功能恢复时间及住院时间明显低于对照组,差异有显著性(P<0.05)。观察组术后各时间段的疼痛视觉模拟量表(visual analogue scale,VAS)评分均明显低于对照组,并发症总发生率(4.17%)明显低于对照组(16.67%),差异均有显著性(P<0.05)。结论经脐单孔腹腔镜阑尾切除术治疗急性阑尾炎可明显减少术中出血,缩短术后恢复时间,降低患者术后疼痛程度及并发症发生率,值得临床应用。
Objective To analyze the operation skill of transumbilical single-port laparoscopic appendectomy for acute appendicitis and the blood condition and recovery after operation.Method Ninety-six patients with acute appendicitis treated in our hospital from January 2017 to January 2019 were divided into observation group and control group by random number table method.The control group was treated by traditional open appendectomy,while the observation group was treated by transumbilical single-hole laparoscopic appendectomy.The differences of clinical indicators,pain degree and complications between the two groups under different surgical treatments were compared.Result Compared with the control group,the observation time was significantly higher than that of the control group(P<0.05).The time of bleeding,the time of anal exhaust,the time of postoperative feeding,the recovery time of gastrointestinal function and the length of hospital stay were compared.In contrast,the observation group was significantly lower than the control group(P<0.05);the pain level was compared,the VAS scores of the observation group were significantly lower than those of the control group(P<0.05);the complications were observed,and the complications of the observation group always occurred.The rate was significantly lower than the control group(P<0.05).Conclusion In the clinical treatment of acute appendicitis,the use of transumbilical single-port laparoscopic appendectomy can significantly reduce intraoperative bleeding,shorten the recovery time,reduce the degree of pain and complications after operation,and has significant clinical application value.
