摘要
目的 探讨血浆、胃液环磷酸腺苷 (c AMP) ,环磷酸鸟苷 (c GMP) ,c AMP/ c GMP(CA/ CG)比值测定对鉴别良恶性胃病的临床意义。方法 对 1 0 7例因胃肠道症状接受胃镜检查配合活检确诊 ,分为慢性胃炎组 50例 ,溃疡病组 2 5例 ,胃癌组 32例 ,用放免法分别测定血浆、胃液 c AMP、 c GMP、 CA/ CG比值。结果 32例胃癌患者测定血浆 c GMP5.88± 1 .0 3pmol/ ml CA/ CG3.80～ 3.31及胃液 c GMP1 .0 3± 0 .1 7pmol/ ml,CA/ CG比值 2 .78～ 2 .2 9。与慢性胃炎组及溃疡病组相比 P值均 <0 .0 1 ,有显著性差异。血浆及胃液 c AMP分别及 1 8.70± 1 .38pmol/ ml及 2 .66±0 .57pmol/ ml与各组相比无差异 (P>0 .0 5)。结论 测定体液 c GMP及 CA/ CG辅助鉴别良恶性胃病有重要临床意义 ;对癌前状态高危人群定期检测环核苷酸可望提高早癌检出率 ;
Objectives:To evaluate the clinical significance of the changes of cAMP and cGMP in both plasma and gastric juice from the patients with gastric disorder in differentiating benign or malignart diseases.Methods:Of 107 patients with gastrointestinal symptoms as diagnosed by endoscope and biopsy examination,chronic gastnitis was 50 cases,peptic ulcer was 25 cases and gastric cancer was 32 cases.All patients were tested for cAMP and cGMP in both plasma and gastric juice by RIA.Results:The value of cGMP of 32 patients with gastric cancer (GC)was 5 88±1 03pmol/ml,CA/CG was 3 80～3 31 in plasma,and 1 03±0 17 pmol/ml,CA/CG was 2 78～2 29 in gastric juice,which were significantly different from the value of patients with chronic gastritis and peptic ulcer(P<0 01).The value of cAMP was 18 70±1 38 pmol/ml in plasma;and 2 66±0 57pmol/ml in gastric juice.No difference of cAMP values was found between compared groups(P>0 05).Concluslons:The test of cGMP and CA/CG is of great clinic significance for the differentiation of benign or malignant gastric diseases;and is helpful for the early diagnosis GC of the people with high risk of GC;Besides,they may be one of the important sign indicating the recurrence of GC after operation.
