摘要
目的 探讨糖尿病患者左心房血流动力学改变及其临床意义。方法 应用彩色多普勒超声心动图对比观测了 65例糖尿病患者及 40例正常人的左心房功能各项指标。结果 糖尿病组与正常对照组比较其各项指标均有显著性差异 (P <0 .0 0 1或P <0 .0 5 )。左房内径 (LADd)、左房压力 (LAP)、左房容积 (LAV)、左房张力 (LAT)、左房射血时间(LAET)和左房射血力 (F)均增加 ,而射血前期 (LAPEP) ,LAPEP/LAET比值、舒张期二尖瓣血流峰速比 (VE/VA)和肺静脉舒张期峰值速度 (Dp)降低。结论 ①糖尿病患者左房收缩功能的代偿性明显增加 ,以维持一定的左房室压差 ,对增加左室充盈量起重要作用 ;
Objective To study the changes of left atrial hemodynamics of patients with diabetes mellitus by color Doppler echocardiongraphy and its clinical value.Methods Left atrial functions were evaluated by color Doppler echocardiography in 65 patients with diabetes mellitus and 40 normal controls.Resultes The results showed that indexes of left atrial funtction in diabetes mellitus group were significantly different compared with that of control group (P<0.001 or P<0.05).Left atrial end-diastole dimension (LADd), left atrial pressure(LAP),left atrial volume(LAV),left atrial tension(LAT),left atrial ejection time(LAET),left atrial ejection force(F) of diabetes mellitus group were more increased,but the left atrial pre-ejection period(LAPEP),the ratios of V E/V A of mitral flow velocity and LAPEP/LAET,the pulmonary vein peak flow velocity during diastole were significantly decreased in patients with diabetes mellitus.Conclusion ① In diabetes mellitus,left atrial systolic function is compensatoryly enhanced to keep constain atrial-ventricular pressure gradient that is important to increase left ventricular filling.②.Color Doppler echocardiography is helpful in detecting the changes of left atrial hemodynamics of patients with diabetes mellitus.
