摘要
目的 了解血浆IGF -1水平与 2型糖尿病神经病变 (DNP)的关系。方法 观察 2型DM患者 115例 ,并依据合并神经病变的严重程度分为 0级DNP组 2 2例 ,1级DNP组 5 6例 ,2级DNP组 2 5例 ,3级DNP组 12例。观察对象分别测定体重指数、FBS、空腹C肽 (CP0 )、餐后 2hC肽 (CP2 )、糖化血红蛋白 (GHbA1C)及IGF -1,并行肌电图检查。血浆IGF -1采用放免法测定 ,各组间差异显著性使用单因素方差分析。结果 血浆IGF -1水平与DNP的严重程度呈显著负相关 (P <0 0 5 ) ;1,2 ,3级DNP组血浆IGF -1水平 [分别为 ( 117 48± 40 3 4 ) μm/ml,( 115 .61± 3 5 .48) μg/ml,( 10 0 .87± 2 0 .42 ) μg/ml]显著低于 0级DNP组 [( 14 5 42± 5 7.70 ) μg/ml,分别为P <0 .0 5 ,P <0 .0 5 ,P <0 .0 1] ;而 1,2 ,3级DNP组间血浆IGF -1水平差异无显著性。结论 循环中IGF -1水平降低可能参与了DNP的发生及发展。
Objective To evaluate the possible relationship between plasma insulin-like growth factor-1 (IGF-1) and diabetic neuropathy (DNP) in type 2 diabetic patients. Methods Plasma concentrations of IGF-1, FBS, GHbA 1C, fast C-peptide and postprandial C-peptide were measured in 115 cases of type 2 diabetes (22 with grade 0 DNP, 56 with grade 1 DNP, 25 with grade 2 DNP, 12 with grade 3 DNP). Muscular electrometry was also performed. Plasma concentrations of IGF-1 were measured by immunoradioassay analysis. Results Plasma IGF-1 levles inversely correlated with the grades of DNP in type 2 diabetic patients ( P<0.05 ) . Patients with grade 1, 2 or 3 of DNP had significant lower plasma IGF-1 concentration [(117.48±40.34) μg/ml, (115 61±35.48) μg/ml and (100.87±20.42) μg/ml, respectively] than that of grade 0 (145.42±57.70) μg/ml ( P<0 05 ,P<0.05,P<0.01 respectively), while plasma IGF-1 concentrations had no significant differences among groups of grade 1, 2 or 3. Conclusion Decreased plasma IGF-1 in type 2 diabetic patients may contribute to the pathogensis of neuropathy.
