摘要
目的：探讨迈之灵治疗老年阴囊水肿的有效性及安全性。方法采用两组平行对照的临床观察研究方法，将82例老年阴囊水肿患者应用计算机随机分为两组，每组41例。对照组给予物理治疗，并口服呋塞米片20 mg，2次/ d。治疗组在对照组基础上口服迈之灵450 mg，2次/ d，10 d 后改为300 mg、2次/ d 口服，疗程共20 d。对比两组患者阴囊体积减小程度、疼痛与不适评分的变化、阴囊溃疡愈合时间等，并对两组的临床疗效进行比较。结果经过20 d 治疗，治疗组阴囊直径为（7.81±0.64）cm，较治疗前（13.30±0.73）cm 明显减小，差异有统计学意义（t =2.201，P ＜0.05）；对照组治疗前后阴囊直径分别为（13.25±0.71）、（10.71±0.65）cm，差异有统计学意义（t =2.369，P ＜0.05），治疗组治疗后阴囊直径减小程度较对照组好（t =2.304，P ＜0.05）。治疗组治疗后阴囊溃疡愈合率为100％（11/11），愈合时间为（14.1±1.8）d；对照组愈合率为70.0％（7/10），愈合时间为（17.1±1.9）d，两组阴囊溃疡愈合率、愈合时间比较差异均有统计学意义（χ^2=4.880，P =0.030；t =2.713，P =0.016）。治疗组与对照组治疗前的临床症状评分比较差异均无统计学意义（P 均＞0.05），治疗后治疗组疼痛与不适、排尿症状、生活质量评分均低于对照组[（7.35±3.08）、（8.47±3.07）分，（3.43±1.91）、（4.52±2.19）分，（5.89±2.11）、（6.96±2.13）分]，差异均有统计学意义（t 值分别为2.260、2.382、2.319，P 均＜0.05）。临床效果评定，治疗组总有效率为87.8％（36/41），对照组为65.8％（27/41），两组比较差异有统计学意义（χ^2=4.580，P ＜0.01）。治疗过程中，两组均未出现不良反应。结论迈之灵片与利尿剂同时使用有协同作用，能明显改善老年阴囊水肿患者的临床症状，消除水肿，促进阴囊溃疡创面的愈合，减轻疼痛
Objective To evaluate the efficacy and safety of Aescuven forte on treating elderly patients with scrotal edema. Methods Eighty-two patients with scrotal edema were randomly divided into control group and treatment group（41cases for each group）. Patients in the control group received physical therapy as well as oral Furosemide tablets for 20 mg and 2 times/ d,while in treatment group were given 450 mg oral Aescuven forte （2 times/ d）,and 300 mg Aescuven forte at 10 d besides the treatment as the control group. The total treatment period was 20 days. The information of scrotal volume,score of discomfort or pain,scrotal ulcer healing time were collected. Results After 20 d treatment,the scrotal diameter in treatment group was（7. 81 ± 0. 64）cm,shorter than that before treatment（13. 30 ± 0. 73）cm,and the difference was statistically significant（ t = 2. 201,P〈 0. 05）. Scrotal diameter of the control group before and after treatment were（13. 25 ± 0. 71）,（10. 71 ± 0. 65） cm,and there were statistically significant differences（ t = 2. 369,P 〈 0. 05）. After treatment,the decreases degree of scrotal diameter in the treatment group was higher than control group（ t = 2. 304,P 〈0. 05）. After treatment,scrotal ulcer healing rate,healing time in treatment groups were 100%（11 / 11）and（14. 1 ± 1. 8）d, and 70%（7 / 10）and（17. 1 ± 1. 9）d in control group,and there were significant difference（ χ^2 = 4. 880,P= 0. 030;t = 2. 713,P = 0. 016）. There was no significant difference in terms of the clinical symptom score between two group before treatment（ P 〉0. 05）. After treatment,the level of pain and discomfort,voiding symptoms,life quality score were（7. 35 ± 3. 08）,（3. 43 ± 1. 91）,（5. 89 ± 2. 11）,lower than those in the control group（（8. 47 ± 3. 07）,（4. 52 ± 2. 19）,（6. 96 ± 2. 13））,and the differences were statistically significant （t = 2. 260,2. 382,2. 319,P 〈0. 05）. The total effective rate of treatment group was 87. 8%�
