摘要
目的探讨乳腺癌X线摄影特征表现与血管内皮生长因子(VEGF)表达的相关性,并分析其临床意义。方法收集经乳腺钼靶X线检查、手术、病理证实的乳腺癌患者,双盲法按照ACR创立并推荐的BI-RADS,将88例(88/91)分为无钙化组和微小钙化组,将55例(55/91)肿块分为有毛刺组和无毛刺组,用免疫组织化学检测各组VEGF的表达的水平及其与微小钙化、毛刺征的关系。结果微小恶性钙化者40例,占44.0%(40/91);55例(60.4%,55/91)肿块患者,边缘有毛刺征49例,占89.1%;无毛刺肿块6例,占10.9%。91例乳腺癌组织中HE染色微小钙化灶表现为成簇分布深色颗粒,VEGF总阳性表达率为79.1%。乳腺癌X线特征表现与VEGF表达的关系中,直接征象、肿块形状阳性与阴性差异无统计学意义(P>0.05),肿块边缘、钙化情况阳性与阴性差异有统计学意义(P<0.05)。结论乳腺癌X线摄影特征表现与VEGF表达有一定关联性,在一定程度上反映VEGF表达状态,为判断肿瘤生物学行为、评价患者预后及指导治疗提供一定的参考。
Objective To investigate the correlation among the mammographic features and the expression of VEGF in breast carcinoma. Methods Ninty one cases of breast cancer underwent mammography and operation,all of them were proved by pathologic examination. According to BI-RADS, 88(88/91) cases were divided into two groups, nonmicrocalcification group and microcalcification group, and 55(55/91) masses were divided two groups,spiculation group and nonspiculation group. The correlations between expression of VEGF and microcalcification or spiculation were studied. Results Expression of VEGF in masses with spicular margin was higher than those without spiculation(χ2=3.985). Expression of VEGF in foci without calcification was significantly higher than the foci with malignant calcifications(χ2=15.643). Conclusion The mammographic features of the breast carcinoma had correlation with expression of VEGF, which refleted the expression of VEGF in breast carcinoma. Thus the biological behavior can be assessed by mammographic features, which provide information for prognosis and therapy.
