精神分裂症伴发代谢综合征患者血清视黄醇结合蛋白4水平的研究 被引量:2

摘要 目的探讨精神分裂症伴发代谢综合征(MS)患者血清视黄醇结合蛋白4(RBP4)水平及其与MS的关系。方法测定诊断为MS的精神分裂症患者(病例组,n=40)血清RBP4水平、体质量指数(BMI)、空腹血糖(FPG)、血脂、血压,以TyG指数评定受试者胰岛素抵抗水平,同时与不伴MS的精神分裂症患者(对照组,n=40)进行对照。结果①与对照组比较,病例组血清RBP4水平显著增高(t=4.856,P〈0.01);②以血清RBP4的中位数分组,血清RBP4水平高值组MS及MS各组分异常(包括肥胖、高血糖、血脂紊乱、高血压)的发生率显著高于血清RBP4水平低值组(χ^2=5.333—24.261,P〈0.05~0.01),高血清RBP4水平与MS及MS各组分异常的发生存在关联性(OR=3.0-12.0,95%CI=1.2~34.5);③血清RBP4水平与TyG指数、BMI、FPG、TG正相关(r=0.319-0.394,P〈0.05~0.01),与HDL-C负相关(r=-0.333,P〈0.05)。结论精神分裂症伴发MS患者血清RBP4增高,并可能与精神分裂症患者中MS的发生有关。 Objective To investigate the level of retinol binding protein -4 (RBP4) in schizophrenia with metabolic syndrome(MS) and explore its relationship with the pathogenesis of MS. Methods Schizophrenia with or without MS were enrolled as cases ( n = 40) or controls ( n = 40). The serum retinol-binding protein 4, metabolic syndrome components including body mass index ( BMI), fasting plasma glucose (FPG), blood-lidpoids and blood pressure were assayed. TyG index was used for insulin resistance assessment. Results The serum level of RBP4 was significantly higher in cases than that in controls ( t = 4. 856, P 〈 0.01 ). When the distribution of RBP4 was stratified into dimidius, the incidence of MS, obesity, hyperglycemia, dyslipidemia and hypertension were significantly increased in subjects with higher RBP4 level than those with lower level (χ^2 = 5. 333 - 24. 261,P 〈 0.05 - 0.01 ). It was showed that increased serum RBP4 was not only associated greater risk for MS, but also significantly with risk for abnormal status of component of MS e. g. obesity( OR =3.0 - 12.0,95% CI = 1. 2 - 34.5 ). There were significantly positive correlations between RBP4 level and TyG index, BMI, FPG or TG( r = 0. 319 - 0. 394, P 〈 0.05 - 0.01 ) and significantly negative correlations between RBP4 level and HDL - C ( r = -0. 333, P 〈 0.05 ). Conclusion The level of RBP4 is increased in schizophrenia patients with MS,which may be a role of RBP4 in the pathogenesis of MS.
