摘要 目的探讨2型糖尿病患者不同性别及甲状腺功能状态下血脂的特点。方法收集279例2型糖尿病患者,依据性别、促甲状腺激素(TSH)水平分组后,比较各组血脂的差别,应用相关分析研究TSH与血脂的关系。结果(1)女性TSH、总胆固醇(TC)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)和载脂蛋白A1(apoA1)高于男性(P<0.05)。(2)亚临床甲状腺功能减退患者的血脂高于甲状腺功能正常者(P<0.05)。(3)甲状腺功能正常的2型糖尿病患者,与TSH<2.5mU/L组相比,TSH为2.5~4.0mU/L组男性HDL-C降低,女性TC升高(P<0.01)。(4)2型糖尿病患者无论男女TSH均与TC、三酰甘油(TG)呈正相关(P<0.05)。结论2型糖尿病患者甲状腺功能与血脂水平密切相关,对血脂异常应注意甲状腺功能监测。 Objective To explore the characteristics of lipid level at different thyroid function status in pa-tients with type 2 diabetes mellitus (T2MD) by gender.Methods 279 patients with T2DM were enrolled and divid-ed into groups according to the thyroid stimulating hormone (TSH) and sex.The lipid level was measured and the cor-relation between TSH and lipid profile was analyzed .Results The serum TSH,total cholesterol(TC),high-density lipoprotein cholesterol ( HDL-C) ,low-density lipoprotein cholesterol ( LDL-C) and apoA1 in female group were signifi-cantly higher than those of male group (P〈0.05).Among euthyroid patients with T2MD,the male group with TSH 2. 5~4.0 mU/L had a lower HDL-C while female group had a higher TC (P〈0.01) compared to TSH 〈2.5 mU/L group.TSH was positively correlated with serum TC and triglyceride ( TG ) , both in male and female patients withT2DM.Conclution The thyroid function is closely associated with lipid level in patients with T 2DM.TSH Moni-toringis is beneficial to lipid control and prevention of its complications .
