摘要
目的了解五家渠市住院糖尿病患者中甲状腺功能异常的发病率及两者之间的相关性,以指导临床工作。方法收集2015年1月—2016年6月在本院内分泌科住院的糖尿病患者438例,并选择同期健康体检者200例为对照组,检测上述人群入组时的甲状腺功能(血清游离T3、血清游离T4、血清STSH、甲状腺过氧化物酶抗体(TPOAb))、肝肾功能、空腹血糖及空腹胰岛素和糖化血红蛋白水平;糖尿病合并甲状腺功能异常患者通过半年治疗、甲状腺功能恢复正常后再次复测上述检验项目。结果 200例健康志愿者甲状腺功能异常发生率为9.5%,而438例2型糖尿病患者甲状腺功能异常者发生率为33.8%,两者差异有统计学意义(P <0.05)。其中临床甲减、亚临床甲减、临床甲亢、亚临床甲亢、TPOAb的发病率分别为5.3%(23/438)、13.2%(58/438)、3.2%(14/438)、4.1%/(18/438)、8.0%(35/438)。且甲减发生率明显高于甲亢(18.5%,7.3%),其中亚临床甲减最为多见(13.2%)。另女性糖尿病患者中甲状腺功能异常的发病率(24.2%,106/438)明显高于男性(9.6%,42/438),差异有统计学意义(P <0.05)。且甲亢和甲减患者经治疗半年、甲功恢复正常后,检测胰岛功能均有所恢复,胰岛素抵抗指数降低。结论糖尿病患者中甲状腺功能异常发生率明显升高,甲状腺功能恢复正常则有利于糖尿病患者血糖的控制。糖尿病患者应常规检查甲状腺功能。
Objective To investigate the incidence of abnormal thyroid function and the correlation between them in hospitalized diabetic patients in Wujiaqu of Xinjiang, so as to guide the clinical work. Methods From January 2015 to June 2016, 438 patients with diabetes mellitus were enrolled in the Department of Endocrinology in our hospital, and 200 healthy persons were selected as the control group. The thyroid function(serum free T3, serum free T4, serum STSHs and TPOAb), liver-kidney function, fasting blood glucose, fasting insulin and glycosylated hemoglobin were measured. The patients with diabetes mellitus complicated with thyroid dysfunction were treated for half a year and all above indexes were measured again after the thyroid function returned to normal. Results The incidence of thyroid dysfunction was 9.5% in 200 healthy volunteers and 33.8% in 438 type 2 diabetic patients(P < 0.05). The incidence of TPOAb in clinical hypothyroidism, subclinical hypothyroidism, clinical hyperthyroidism and subclinical hyperthyroidism were 5.3%(23/438), 13.2%(58/438), 3.2%(14/438), 4.1/(18/438)or 8.0%(35/438), respectively. The incidence of hypothyroidism was significantly higher than that of hyperthyroidism(18.5%), and subclinical hypothyroidism was the most common(13.2%). The incidence of thyroid dysfunction in female patients with diabetes mellitus(24. 2/106/438) was significantly higher than that in males(9. 6% 42/438)(P < 0. 05). After half a year of treatment, the pancreas islet function of hyperthyroidism and hypothyroidism recovered and the insulin resistance index decreased. Conclusion The incidence of abnormal thyroid function in diabetic patients is significantly increased, and the recovery of thyroid function is beneficial to the control of blood glucose in diabetic patients. Patients with diabetes should be routinely examined for thyroid function.
