摘要
目的:检测全身炎症反应综合征(SIRS)患儿血清TNF-α浓度变化,探讨促炎介质和抗炎介质对SIRS患儿疾病转归的影响。方法:选择SIRS患儿42例作为实验组,其中发生多器官功能障碍的归入MODS组,其余归为非MODS组;健康体检儿童30例作为对照组。采用酶联免疫吸附法(ELISA)测定实验组入院第1、3、5 d和对照组血清中TNF-α及PGE2水平。结果:组间比较:SIRS组TNF-α和PGE2血清水平明显高于对照组(P<0.01);MODS组第1、3、5 d TNF-α和PGE2血清水平明显高于非MODS组(P<0.01)。组内比较:非MODS组TNF-α和PGE2血清水平于入院第3 d达高峰,第5 d呈下降趋势。MODS组TNF-α和PGE2血清水平于入院第1、3、5 d呈逐渐升高趋势,且始终维持在较高水平,第5 d达峰值(P<0.01)。结论:①SIRS患儿血清TNF-α及PGE2水平动态变化,持续升高提示预后不良。②血清TNF-α水平可以作为临床监测炎症反应程度的指标,血清PGE2水平对临床监测抗炎反应程度有参考价值。
Objective: To determining the change of serum levels of TNF - α and PGE2 in SIRS children patiens; Probe into the pro- inflammatory mediators and anti - inflammatory influence the transform of SIRS children. Methods:42 example SIRS children patients as experimental group, divided into two groups: MODS and non- MODS, Multiple organ dysfunction belong the MODS group, the rest own by a non- MODS group. The service physical examination child 30 examples as the control group. Determination of TNF - α and PGE2 serum levels of the control group and experiment group the first, the third and the fifth day by enzyme - linked immunosorbent assay(ELISA). Results: Comparison between groups: the serum level of TNF - α and PGE2 in the SIRS group are significantly higher than the control group(P 〈 0.01) ;the serum level of TNF- α and PGE2 on the first,the third and the fifth day in the MODS group is increased significantly than the non- MODS group (P 〈 0.01 ). Group comparison: the serum level of TNF - a and PGE2 in the non - MODS group has a peak in the third day, and there has been a declining trend since the 5th day. In the MODS, the serum level of TNF-α and PGE2 on the first,the third and the fifth day group show a trend of rising, but a relative high level is still maintained, a peak in the fifth day( P 〈 0.01). Conclusion: ①The serum level of TNF-α and PGE2 is becoming dynamic change, and continue to rise indicates poor prognosis in SIRS children. ②The serum level of TNF - α could be a indicator for Clinical monitoring the degree inflammatory response, the serum level of PGE2 have a reference value for Clinical monitoring the anti - inflammatory response degree.
