摘要 目的探讨手指伸肌和侧韧带正常结构及损伤的MRI表现。方法选取正常志愿者19名和肌腱韧带损伤患者23例。所有患者及志愿者均行手指肌腱韧带MR检查,将手指损伤患者与正常志愿者的MRI表现进行影像对照,分析损伤的MRI表现特点。结果19名志愿者手指肌腱韧带MRI表现为T1WI和质子密度脂肪抑制(PD—FS)序列均为细条带状均匀低信号。23例手指肌腱韧带损伤患者(其中侧韧带损伤13例,中央束损伤4例,末端腱损伤5例,矢状束损伤1例)主要表现为损伤韧带T1WI序列信号不清、纤维连续性中断,PD—FS序列信号不均匀增高,损伤周围可见软组织水肿。结论MR检查是明确手指肌腱韧带正常结构及损伤的有效方法,在手指肌腱损伤的诊断和治疗方面具有重要的临床应用价值。 Objective To explore the MRI features of the anatomy and injuries of extensor mechanism and collateral ligament of the fingers. Methods There were 23 patients with ligaments and tendons injury in the fingers and 19 volunteers with the normal anatomy without injuries. Twenty-three patients and 19 normal volunteers were studied with MR imaging. The MR imaging findings of patients were compared with normal volunteers. Results The normal ligaments and tendons of fingers in 19 volunteers showed low signal intensity on MR. Twenty-three patients with injures of ligament and tendon of fingers, 13 cases with collateral ligament injury4 cases with the central slip injury,5 cases with terminal tendon injury, 1 case with sagittal band injury, and the ligaments and tendons disruption manifested as increased signal intensity and demonstrated as poor definition, discontinuity and heterogeneously of the involved ligament and tendon. There was edema in the soft tissue. Conclusions MR imaging is an accurate method for evaluation of the anatomy and pathologic conditions of the finger, and it is a useful tool for diagnosis and treatment.
