摘要
目的探讨黏附因子CD44、C-erbB-2基因(HER-2)、抑癌基因突变型P53(MtP53)、血管内皮生长因子(VEGF)在非小细胞肺癌(NSCLC)中的表达及与临床病理特征、预后的关系。方法选取术后经病理证实的40例NSCLC及10例肺良性病变组织。采用免疫组化Envision法检测肿瘤相关基因蛋白的表达。采用χ2检验分析相关基因在NSCLC及良性组织中表达的差异及NSCLC中相关基因表达与临床病理特征的相关性。采用Kaplan-Meier生存曲线分析相关基因在NSCLC组织中的表达与预后的关系。结果检测出CD44、HER-2、MtP53、VEGF在NSCLC组织中高表达率分别为65%、45%、50%、55%,显著高于对照组(P<0.05)。CD44与病理类型有关(P<0.05);HER-2与年龄、淋巴结分期有关(P<0.05);MtP53与性别、吸烟史、淋巴结分期、分化程度、TNM分期有关(P<0.05);VEGF与病理类型、淋巴结、分化程度、肿瘤大小、TNM分期有关(P<0.05)。Kaplan-Meier生存分析提示CD44、HER-2、MtP53、VEGF基因阳性表达的NSCLC患者预后差(P<0.05)。结论 CD44、HER-2、MtP53、VEGF蛋白在NSCLC组织中呈高表达,与肿瘤发生、发展有关;CD44、HER-2、MtP53、VEGF阳性表达者预后差,可作为判断NSCLC预后的参考指标。
Objective To investigate the relationship of expression of CD44, HER-2, P53, and VEGF in non small cell lung cancer(NSCLC)and clinical pathological feature. Methods 40 NSCLC and 10 pulmonary benign disease patients were enrolled. Envision technology was used for detection of NSCLC related protein. Chi-squared analysis was used between groups. Kaplan-Meier learning curve was used for prognosis analysis. Results The high expression ratio for CD44, HER-2, MtP53 and VEGF in NSCLC were 65%, 45%, 50% and 55% which showed significant difference when compared control group(P〈0.05). Our results showed high expression of CD44 was related to pathology(P〈0.05). High expression of HER-2 was related to age and lymph node stage(P〈0.05). High expression of P53 was related to sex, smoke history, lymph node stage, differentiation and TNM stage(P〈0.05). High expression of VEGF was related to pathology, lymph node, differentiation, size of tumor and TNM stage(P〈0.05). Kapaln-Meier survival analysis showed worse prognosis for NSCLC patients with expression of CD44, HER-2, MtP53 and VEGF(P〈0.05). Conclusion High expression of CD44, HER-2, MtP53 and VEGF were related to development of NSCLC. Patients with high expression of CD44, HER-2, MtP53 and VEGF got poor prognosis. They could be considered as important prognostic factors for NSCLC.
