摘要
目的探究老年2型糖尿病患者糖化血红蛋白(HbAl c)、血脂与尿微量白蛋白(mALB)之间的关系。方法我院于2011年2月至2013年1月共收治了96名老年2型糖尿病患者,设置为糖尿病组,将74名健康的老年者设置为对照组,对两组患者的mALB、HDL-C、LDL-C、TG、Ch、GLU进行了检测。糖化血红蛋白的检测采用的是免疫比浊法;尿微量白蛋白的测定采用的是免疫比浊分析方法,低密度脂蛋白胆固醇(LDL-C)以及高密度脂蛋白胆固醇(HDC-C)采用的是直接酶法;胆固醇的检测采用的是胆固醇氧化酶法;空腹血糖(GLU)的检测采用的葡萄糖氧化酶法;三酰甘油(TG)的检测采用的甘油磷酸氧化酶法。结果通过对检测结果进行分析,发现mALB、LDL-C、TG、Cho、GLU、HbAl c的浓度,糖尿病组患者要明显高于对照组(P<0.01),而HDL-C的血清浓度糖尿病组患者要明显低于对照组(P<0.01)。当患者体内糖化血红蛋白水平有所增高时,尿微量白蛋白以及空腹时血糖也会随之增高,二者之间呈现显著的正相关(r=0.648,P<0.01;r=0.483,P<0.01)。结论老年2型糖尿病患者体内尿微量白蛋白的增高程度与糖化血红蛋白的增高相关,并且尿微量白蛋白是糖尿病患者早期肾脏损害的一项灵敏指标。可以通过检测mALB以及HbAl c来早期诊断和治疗糖尿病肾病。
Objective To detect the HbA1c, blood-lipid and urinary microalbumin(U-mAlb) in elderly patients with type 2 diabetes, and observe their relationships. Methods 96 elderly patients with type 2 diabetes(diabetic group),74 healthy elderly persons as control(control group). The levels of GLU, CH, LDL-C, HDL-C and U-mAlb were tested in two groups. The HbA1c was detected by immunoturbidimetry;U-mAlb was tested by immunoturbidimetry;GLU, Ch, TG were tested respectively by the GLUcose oxidase method, cholesterol oxidase method, and glycerophosphoric acid oxidase method;LDL-C and HDL-C were measured by the direct enzyme method. Results The levels of HbA1c, GLU, CH, LDL-C, HDL-C and U-mAlb in diabetic group were signiifcantly higher than that of the control group(P〈0.01), the level of HDL-C was signiifcantly lower than that of the control group (P〈0.05). The levels of GLU and U-mAlb increased along with the HbA1c. The levels of HbA1c were positively associated with the levels of U-mAlb (P〈0.01). Conclusion The increase degree of U-mAlb is related to the HbA1c, and the U-mAlb is the sensitive index of early renal damage in diabetes mellitus. There is important signiifcance to diagnose and treat early for diabetic nephropathy by combined detection of HbA1c and UmAlb.
