摘要
目的分析糖化血红蛋白、空腹血糖、尿微量白蛋白检测与糖尿病肾病的关系。方法选择2014年7月至2018年7月在我院治疗的糖尿病肾病患者93例,糖尿病非肾病患者97例及体检正常者50例,分别检测各组受试者的血红蛋白、尿微量白蛋白、空腹血糖的水平。结果 DN患者与糖尿病非肾病患者以及正常体检者相比,HbA1c、FBG、尿mAlb的水平均明显偏高;糖尿病非肾病组患者HbA1c、FBG、尿mAlb的水平也明显高于正常体检者,差异具有统计学意义(P <0. 05)。HbAlc大于9. 0%的37例糖尿病患者中DN的检出率为49. 3%,明显高于HbAlc小于6. 5%和HbAlc在6. 5%~9. 0%之间的DN患者的检出率,差异具有统计学意义(P <0. 05);HbAlc小于6. 5%和HbAlc为6. 5%~9. 0%的DN患者的检出率相比差异不具有统计学意义(P>0.05);FBG大于11. 1 mmol/L的56例糖尿病患者中DN的检出率达33. 4%,明显高于FBG小于7.8 mmol/L的受试者的DN检出率;FBG为7. 8~11.0 mmol/L的105例糖尿病患者中DN的检出率达25.7%,明显高于FBG小于7.8的受试者的DN检出率;FBG大于11.1 mmol/L的患者DN的检出率与FBG为7.8~11.0mmol/L的DN患者的检出率相比差异无统计学意义(P>0. 05)。结论对糖尿病患者血红蛋白、空腹血糖、尿微量白蛋白的测定能为糖尿病肾病的早期诊断及治疗提供一定的参考依据。
Objective To explore the relationship between HbAlc,FBG and mAlb levels and diabetic nephropathy (DN). Methods We selected 93 cases of DN,97 cases of non-DN and 50 cases of normal healthy physical examinees for the study and detected the levels of HbAlc,FBG and mAlb in all patients. Results The levels of HbAlc,FBG and U-mAlb of DN patients were significantly higher than non-DN diabetic patients and normal personnel,and the levels of HbA1c,FBG and U-mAlb of non-DN diabetic patients were also significantly higher than normal personnel ( P <0.05).The detection rate of DN in patients with HbAlc greater than 9% was 49.3%,which was significantly higher than that of DN in patients with HbAlc less than 6.5% and HbAlc within 6.5%-9.0%( P < 0.05).There was no significant difference between the DN detection rates in patients with HbAlc less than 6.5% and HbAlc within 6.5%-9.0%( P > 0.05).The detection rate of DN in 56 diabetic patients with FBG greater than 11.1 mmol/L was 33.4%,significantly higher than the DN detection rate in the group with FBG less than 7.8 mmol/L.The detection rate of DN in 105 cases of diabetes mellitus with FBG within 7.8-11.0 mmol/L was 25.7%,significantly higher than the DN detection rate in the group of FBG less than 7.8 mmol/L;.There was no significant difference in the detection rate of DN between the 105 diabetic patients with FBG 7.8-11.0 mmol/L and the detection rate of DN in 49 diabetic patients with FBG greater than 11.1 ( P >0.05). Conclusion The determination of HbAlc,FBG and mAlb can provide a reference for the early diagnosis and treatment of DN.
