摘要
目的 观察普通人群接种病毒性乙型肝炎（乙肝）疫苗后免疫效果及安全性。方法 选择16~49岁成年人3178人，接种10 μg乙肝疫苗3剂，采集免疫前后2份血清，第1次采血与接种第1针疫苗同步进行，第2次采血在全程接种后1个月。运用化学发光法定量检测乙肝病毒表面抗原（HBsAg）、抗乙肝病毒表面抗体（抗-HBs）和抗乙肝病毒核心抗原抗体（抗-HBc）。结果 免疫前抗-HBs阴性者接种疫苗后阳转率97.68%，不同年龄组阳转率差异有统计学意义（χ2=17.28，P=0.001）；免疫前抗-HBs阴性者接种后抗-HBs几何平均浓度（GMC）为 1123.31 mIU/ml；免疫前抗-HBs阳性者接种疫苗前后GMC分别为 63.32 mIU/ml和 9415.59 mIU/ml，比较前后GMC差异有统计学意义（t=71.141，P=0.00）。结论 普通人群接种乙肝疫苗后，近期能取得良好的免疫效果，其长期效果有待进一步观察。
Objective To evaluate the immunological effect and safety of hepatitis B vaccine in general population. Methods Totally 3178 adult aged 16 to 49 years were selected to administer 3 doses of hepatitis B vaccine （ 10 μg）. The first blood samples were taken while the first hepatitis B vaccine was given, the second blood samples were taken 1 month after the completion of the series vaccination. All the blood samples were detected for hepatitis B virus （HBV） surface antigen （HbsAg）, antibody to hepatitis B virus surface antigen （anti-HBs） and antibody to hepatitis B virus core antigen （anti-HBc） by using chemiluminescence assay. Results For the people who were anti-HBs negative before vaccination, the positive conversion rate was 97.68%, the age specific differences were statistical significant （X2 = 17. 28 ,P =0. 001 ）. The geometric mean concentration （GMC） of anti-HBs was 1123.31 mIU/ml. For the people who were anti-HBs positive, the GMC of anti-HBs were 63.32 mIU/ml and 9415.59 mIU/ml before and after vaccination, the difference was statistical significant （ t = 71. 141, P = 0. 00）. Conclusion The short term effect of hepatitis B vaccine immunization is good in general population, but the long term effect needs further observation.
