摘要
目的以hVEGF165基因转染的骨髓间充质干细胞移植治疗兔激素性股骨头坏死模型,通过影像学观察、病理学及免疫组织化学检查等方法评价疗效。方法使用马血清与醋酸泼尼松龙制备兔激素性股骨头缺血性坏死动物模型后分为4组,分别进行以下治疗:A组(不作任何处理);B组(髓心减压+自体骨移植);C组(髓心减压+骨髓间充质干细胞+自体骨移植);D组(髓心减压+Ad-VEGF165转染骨髓间充质干细胞+自体骨移植)。术后3周处死行病理学、免疫组织化学检查及Western Blot检测股骨头内VEGF的表达,比较各组间的差异。结果 D组在软骨下血管、VEGF阳性面积百分比、股骨头内VEGF的相对浓度均高于其他各组;在VEGF平均灰度值、空骨陷窝率低于其他各组,且各组间差异有统计学意义。结论转染了Ad-VEGF165的骨髓间充质干细胞在宿主股骨头局部可以表达VEGF蛋白,且较单纯的骨髓间充质干细胞移植表达的强度更高,对激素性股骨头坏死进行早期治疗具有确切疗效。
Objective To use gene transfection technique to construct the BMSCs transfected by VEGF165 and autologous bone graft technique to treat the steroid - induced osteonecrosis in rabbit, and to observe the effect treatment by the method of image and pathology and immunohistochemistry. Methods The method of randomized block design to distribute the models of the steroid - induced osteoneerosis of rabbit that were constructed by injected with horse serum and prednisolone. The rabbits were divided into Group A ( untreated) , Group B (core decompression + autogenous bone graft) , Group C (core decompression + BMSCs + autogenous bone graft) and Group D (core decompression + adenovirus to transfection of BMSCs + autogenous bone graft). After 3 weeks, the VEGF in the femoral head was detected by using the pathology, immunohistochemistry and Western Blot. Results The numbers of blood vessel under cartilage, the relative concentration of VEGF and the area percent of staining of VEGF in group D were significantly higher than that in other groups. At the same time, the threshold of grey scale of VEGF and the empty bone lacuna rate were significantly lower than that in other groups and there was statistical difference among different groups. Conclusion The BMSCs transfected by VEGF165 gene can not only express protein of VEGF at region of the femoral head in rabbits, but also express stronger than onefold BMSCs. This carrier has a curative effect in the steroid - induced avascular necrosis of the femoral head in rabbits.
