摘要
目的:利用皮肤共聚焦扫描显微镜(CLSM)技术对光泽苔藓的CLSM影像学特征进行研究。方法:对37例临床诊断为光泽苔藓的患者,选取典型皮损,先将皮损及皮损附近正常皮肤做CLSM检查,再对该处皮损行组织病理学检查,并将两者进行对比研究。结果:皮损处CLSM影像学特征:真皮乳头层水平扫描拼接图像显示基底细胞环明显扩张,其内真皮乳头明显增宽,可见不等量圆形、椭圆形、不规则形、高折光的噬黑素细胞及其他较为致密的中低折光混合炎性细胞;其次在增宽的真皮乳头处由表皮向下纵向扫描依次可见中等度折光的角化不全,表皮-真皮界面模糊;同时表皮纵向扫描层次较正常皮肤减少。结论:结合临床表现、CLSM影像学特征可为光泽苔藓的诊断及鉴别诊断提供有力的依据。
Objective: To investigate the iconographic characteristics of lichen nitidus by using skin Confocal Laser Scan ning Microscopy (CLSM). Methods: The skin lesions and marginal normal skin of 37 cases clinically diagnosed as lichen nitidus were respectively selected for CLSM and histopathological examinations, and the results obtained by the two methods were compared. Results: Digital spliced image of horizontal scanning in papillary dermis showed an enlarged annular structure of basal layer, in which the width of papillary dermis were obviously increased, oval or irregular melanophages with strong light refraction, and various inflammatory cells with mild or weak light refraction in papillary (or superficial) dermis; Moreover, the longitudinal scanning from the epidermis next to papillary dermis further showed parakeratosis with mild light refraction, blurred epidermal-dermal interface; In addition, the decreased longitudinal scanning layers in skin lesion were observed using CLSM when compared with that of normal skin. Conclusion: In combination with clinical features, the icono- graphic characteristics of CLSM contributed a lot to the clinical and differential diagnosis of lichen nitidus.
