下腰椎术后急性马尾神经损伤综合征6例分析 被引量:1

摘要 背景:下腰椎术后可能发生急性马尾神经损伤,后果严重,应引起骨科和神经科医师的重视。目的:分析下腰椎术后出现急性马尾神经损伤综合征的原因,并探讨其预防措施。方法:2004年5月至2012年2月发生下腰椎术后急性马尾神经损伤综合征6例,回顾分析每例患者马尾神经损伤的临床表现、发生原因及处理方法。结果:4例行再手术治疗,2例保守治疗,随访0.5~2年。1例行血肿清除术后完全恢复,2例行减压融合内固定术后部分恢复,1例行扩大减压术后症状无好转,2例保守治疗者症状无改善。结论:对施行下腰椎手术患者应制定详细的术前评估,预期术中易发生马尾神经损伤患者需减压彻底、避免长时间过度牵拉、彻底止血及术后密切观察,一旦损伤诊断明确,宜早期手术探查,以利于患者神经功能恢复。
