重组人骨形态发生蛋白2诱发黄韧带骨化的病理学研究 被引量:3

摘要 目的 :研究实验性黄韧带骨化的病理演变特点。方法 :将重组人骨形态发生蛋白 2 ( rh BMP-2 )诱发的骨化过程中不同时期的动物黄韧带 ,采用石蜡切片苏木精 -伊红 ( H-E)染色、阿尔辛蓝 ( AB)染色和免疫组化染色 (分别以 S-10 0单抗和胶原 单抗为第一抗体 ) ,然后光镜观察。 结果 :rh BMP-2植入后 ,成纤维细胞代谢活跃 ,出现软骨化生 ,并逐渐增生活跃 ,基质中 型胶原纤维和硫酸软骨素增多 ,4周时达到高峰 ,此后 ,肥大的软骨细胞开始退变 ,基质钙化逐渐明显 , 型胶原纤维和硫酸软骨素随之减少 ,8周时出现编织骨 ,并可见脊髓神经纤维水肿、断裂、脱髓鞘和神经元细胞变性等改变。结论 :rh BMP-2以软骨内骨化方式诱导黄韧带骨化 ;在软骨细胞增生最为活跃时 ,基质内 型胶原和硫酸软骨素的含量最高 ,并随着骨化的成熟逐渐下降 ; Objective: To study the progressing pathological features of experimental ossification of ligamentum flavum. Methods: Ligamentum flavum in different periods of ossification, which were induced by recombinant human bone morphogenetic protein 2 (rhBMP 2), were studied using parrafin slices with H E stain, histopathology stain and immunohistology stain (S 100 monoclonal antibody or type Ⅱcollagen monoclonal antibody as first antibodies) and light microscope. Results: After the implanting of rhBMP 2, fibroblasts were very active in metabolism and differentiated into chondrocytes with increasing proliferation activity. The amount of type Ⅱcollagen fibers and chondroitin sulfuric acid in the matrix increased and reached the hightest on the 4th week. After that, many hypertrophied chondrocytes began degenerate. Calcification of the matrix became more obvious. Type Ⅱcollagen fibers and chondroitin sulfuric acid in the matrix decreased gradually. In the 8th week, woven bone were shown. Edema, broken and demyelination of neurofibers and degeneration of neurons in the spinal cord were seen in this period. Conclusion: rhBMP 2 induces the ossification of ligamentum flavum through endochondral ossification. The amounts of type Ⅱcollagen fibers and chondroitin sulfuric acid in the matrix are the highest when the proliferation of chondrocytes is most active, and decreases gradually with the maturation of ossification. The edema and degeneration of spinal cord can be seen before the appearence of neurological damage.
