摘要
目的 :分析糖尿病知识掌握程度与糖尿病医疗费用的关系。方法 :对 339例 2型糖尿病患者的糖尿病知识掌握程度进行试卷测试、评分 ,并了解糖尿病直接医疗花费。根据糖尿病知识掌握的程度进行分组 ,比较与分析血糖控制水平、并发症检出率和直接医疗花费。结果 :糖尿病知识好的患者平均年龄大、病程长、文化程度和经济收入较高 ,血糖控制较好 ;在糖尿病直接医疗费用中 ,治疗费用减少 ,监测费用增加 ,而总的费用与糖尿病知识较差组无明显差异。结论 :糖尿病教育改变了患者直接医疗费用的构成比 ,总费用虽无明显改变 ,但血糖控制改善明显 ,有利于糖尿病并发症的防治和长期医疗费用的下降。
Objective: To study the role of diabetes education on the direct medical cost in diabetic patients. Method: The relationship between knowledge, behavior on diabetes and economics of diabetes-care delivery was analyzed and evaluated in 339 type 2 diabetic patients by standard questionnaire, which consists of a number of questions on diabetes diet, exercise, medicine, treatment compliance and diabetic complications. All the answers to these questions were marked by the same diabetes educator. The cost for diabetes care and diabetic complications, including drugs and self-monitoring of blood glucose or urine glucose, were recorded based on patients' answers and medical records. Results: blood glucose control was significantly better in these patients (Group A) with better knowledge and skill of diabetes management than in those (Group B) with poor understanding on diabetes. The average cost for monitoring was more, but medical cost for others( mainly for drugs and in-hospital care) was less, in Group A than in Group B. Totally average medical cost in two groups was not significantly different. Conclusion: diabetes education improved diabetes care and had better cost efficiency results in these Chinese diabetic patients.
