摘要
目的观察糖尿病视网膜病变患者眼底荧光素血管造影(FFA)中黄斑中心凹无血管区(FAZ)损害相关影响因素。方法糖尿病视网膜病变患者64例(128眼)纳入研究,受检者均行最佳视力矫正、裂隙灯显微镜检查、裂隙灯前置镜检查及彩色眼底照相、荧光素眼底血管造影检查。测量FFA中黄斑中心凹FAZ的直径,并依据FAZ的直径、FAZ的边界和FAZ周围毛细血管网的损害对环黄斑病变进行分级。采用Spearman相关分析分析FAI直径与FAI的轮廓、FAI毛细血管缺失程度黄斑水肿的类型、糖尿病视网膜病变的分级、糖尿病病史、高血压病史和视力的相关性。结果128眼FAZ平均直径为(604-4-136)仙m。依据FAZ直径分级,1级68只眼,2级58只眼,3级2只眼。Sperman相关分析:FAZ的直径与FAZ的轮廓损害(r=0.197,P=0.026)和黄斑水肿的类型(r=0.192,P=0.030)相关;FAZ的轮廓与FAZ毛细血管缺失的程度(r=0.725,P=0.000)、黄斑水肿的类型(r=0.355,P=0.000)、糖尿病视网膜病变的分级(r=0.245,P=0.005)、视力(r=-0.275,P=0.002)相关;FAZ毛细血管缺失的程度与黄斑水肿的类型(r=0.326,P=0.00)、糖尿病视网膜病变的分级(r=0.275,P=0.002)相关。结论糖尿病视网膜病变患者黄斑区无血管区的损害与糖尿病患者的视力、黄斑水肿的类型和糖尿病视网膜病变的程度相关。
Objective To assess the correlation between the foval dilation of the foveal avascular zone (FAZ) in fluorescein angiography and the related factors in diabetic retinopathy. Methods This study involved 64 patients (128 eyes) with diabetic retinopathy. All the eyes were examined by best corrected visual acuity, slit-lamp ophthalmoscop, fudus photography, fundus fluorescein angiography (FFA) and spectral-domain optical coherece tomography. The diameter of FAZ, the grade of maculopathy by the diameter of FAZ, the outline of FAZ and the capillary loss were measured. The correlations were analyzed. Results All 128 eyes were inrolled in the study ; the average diameter of the FAZ was (604 ± 136) μg. Sperman correlation analysis showed the FAZ diameter was related to the FAZ outline( r = 0. 197, P = 0.026) and types of macular edema( r = 0. 192, P = 0. 030) ; the outline of FAZ was related to foval capillary loss( r = 0. 725, P = 0. 000), types of macular edema( r = 0. 355, P = 0. 000), grades of diabetic retinopathy(r = 0.245, P = 0. 005 ), vision acuity( r = - 0. 275, P = 0. 002) ; foval capillary loss was related to types of macular edema(r = 0. 326, P = 0. 000) and grades of diabetic retinopathy (r = 0. 275, P = 0. 002). Conclusion There is correlation between the FAZ damage and the vision acuity, types of macular edema and grades of diabetic retinopathy.
