人乳头状瘤病毒感染与宫颈病变的相关性研究 被引量:26

Study on correlation between prevalence of human papilloma virus infections and cervical cancerization

摘要 目的调查诸暨市女性宫颈人乳头状瘤病毒(HPV)感染状况,并研究其与宫颈病变的相关性,以探讨预防和治疗宫颈癌的新手段。方法对在诸暨市医院行宫颈筛查的3153名女性,进行HPV、新柏氏液基细胞学技术(TCT)及宫颈组织病理诊断,分析HPV感染状况与宫颈癌变的相关性。结果调查3153例行宫颈筛查的妇女中HPV检出阳性946例,感染率30.0%;3153例行宫颈筛查的妇女中,宫颈正常、宫颈上皮内瘤变Ⅰ(CINⅠ)、宫颈上皮内瘤变Ⅱ-Ⅲ(CINⅡ-Ⅲ)及宫颈癌中HPV检出率分别为25.4%、31.6%、74.7%、97.4%,显示HPV检出率随宫颈病变程度的增加而增加(P<0.05),以宫颈正常组为对照,CINI、CINⅡ-Ⅲ及宫颈癌的OR值分别为1.8、29.8、47.4;CINⅡ-Ⅲ、宫颈癌>二重感染率显著高于宫颈正常组及CINⅠ组,差异有统计学意义(P<0.05)。结论诸暨市女性宫颈HPV感染较高,以HPV16、HPV18和HPV58型为主,HPV感染与宫颈癌的发生高度相关,是宫颈癌发生的高危因素。 OBJECTIVE To investigate the prevalence of human papilloma virus (HPV) infections in women in Zhuji area and explore its correlation with cervical cancerization so as to prevent and treat cervical cancer. METHODS A total of 3153 women who underwent the cervical cancer screening in Zhuji hospitals were collected. The HPV genotyping, TCT and histopathological diagnosis were performed. The correlation between the incidence of HPV and the cervical cancerization was analyzed. RESULTS The overall rate of HPV infections in 3153 women was 30.0% (946 cases). Of 3153 women who underwent the cervical screening, the incidence rate of the HPV infections was 25.4% in the women with normal in normal cervix,31.6% in the women with CIN Ⅰ ,74.7% in the women with CIN Ⅱ-Ⅲ , and 97.4% in the patients with cervical cancer, indicating that the incidence rate of HPV infections was increasing as the severity of cervical lesions increased (P〈0.05). Compared with the women with normal cervix as controls, the OR of CIN Ⅰ, CINⅡ-Ⅲ and cervical cancer were 1. 8, 29. 8, and 47. 4, respectively;the incidence rates of the more than double infections in the patients with CIN Ⅱ-Ⅲ or cervical cancer were obviously higher than that of the women with normal cervix and the women with CINⅠ , the difference was statistically significant (P〈0.05). CONCLUSION The incidence of cervical HPV infections is high in the women of Zhuji area. The HPV16, HPV18, and HPV58 are the prevalent subtypes; the cervical cancer is highly correlated with the HPV infections and is a high-risk factor of the cervical cancer.
