摘要
目的:通过文献研究探讨慢性前列腺炎中医证型与前列腺触诊的相关性。方法:对近30年慢性前列腺炎文献资料进行检索,遵循循证医学原则,应用EXCEL建立数据库,将文献进行二次录入核对。运用SPSS10.0软件对证候及前列腺触诊要素进行频次分析。结果:检索到符合纳入标准的文献55篇,经规范后共获得25个中医证型,出现频次为165次。出现频次前3位的证型分别为湿热蕴结(包括湿热下注),气滞血瘀,肾虚型(包括肾气虚、肾阳虚、肾阴虚)。前列腺触诊内容经规范化后获得12个关键词,出现频次263次。统计分析发现,出现前列腺饱满的55次中,湿热蕴结证型占39次(39/55,70.91%);前列腺结节的42次中,气滞血瘀型占27次(27/42,64.29%);前列腺质硬的30次中,气滞血瘀型占23次(23/30,76.67%);前列腺体积萎缩19次中,肾虚证占15次(15/19,78.95%);腺液少18次中,肾虚证占11次(11/18,61.11%);前列腺质软15次中,肾虚证占9次(9/15,60.00%);腺液多的7次中均为湿热蕴结证;直肠空虚的3次均为肾虚证。结论:慢性前列腺炎中医证型和前列腺触诊有一定的相关性。目前,前列腺触诊命名没有统一,不同学者使用不同的术语。今后应当在前列腺触诊术语的规范化,前列腺触诊的量化和标准化方面做进一步研究。
Objective： To study the correlation between the prostate palpation of chronic prostatitis（CP） patients and its TCM syndromes.Methods： The literatures about CP in past 30 years were collected.The principle of evidence-based medicine was used to establish the databank with EXCEL,and the literature meeting the enrolled criteria were recorded and checked.SPSS 10.0 software was adopted to analyze frequency-times of syndromes and prostate palpation.Results： 25 TCM syndromes were received from 55 articles that met the inclusion criteria,frequency of occurrence was 165 times.The top three syndromes were stagnation of damp-heat,qi stagnation and blood stasis,kidney deficiency.The essence of prostate palpation was in all 12 kinds,frequency of occurrence was 263 times.Damp-heat syndrome type accounted for 70.91%（39/55） in prostate plump.Qi stagnation and blood stasis type accounted for 64.29%（27/42） in prostate nodules,76.67%（23/30） in prostate hard.Kidney deficiency accounted for 78.95%（15/19） in prostate volume atrophy,61.11%（11/18） in gland fluid less.Conclusion： Between TCM syndromes of CP and prostate palpation have certain relevance.At present,the names of prostate palpation are not unified,different scholars use different terminology.So we should thoroughly keep on studying the quantification and standardization of prostate palpation.
