血浆脑钠肽在急性心肌梗死患者中的表达及临床意义 被引量:4

The expression and clinical value of plasma brain natriuretic peptide in patients with acute myocardial infarction

摘要 目的探讨血浆脑钠肽(brain natriuretic peptide,BNP)在急性心肌梗死(acute myocardial infarction,AMI)患者中的表达及临床意义。方法抽取AMI患者外周静脉血,ELISA方法检测BNP水平;行超声心动图及组织多普勒超声检查,评估AMI患者左心室大小、收缩及舒张功能及右心室功能,心电图观察AMI患者梗死部位;分析AMI患者血浆BNP与上述指标的关系。结果 AMI组患者血浆BNP水平明显高于正常人群[(3784.57±634.50)pg/mL vs(68.35±6.97)pg/mL,P=0.000];不同部位AMI患者血浆BNP水平相差不大,差异无统计学意义(P>0.05);AMI组患者左心室射血分数与血浆BNP水平呈明显负相关(P=0.005,r=-0.53);Killip分级为Ⅲ级、Ⅳ级AMI患者的血浆BNP水平明显高于Ⅰ级、Ⅱ级患者,差异有统计学意义(P<0.05)。结论血浆BNP水平可能是评估AMI患者左心室收缩功能的一个重要指标。 Objective To explore expression and clinical value of plasma brain natriuretic peptide (BNP) in patients with acute myocardial infarction(AMI). Methods The peripheral venous blood of patients with AMI were collected, and the BNP levels were detected by ELISA. The left ventricular size, systolic and diastolic function and right ventricular function of AMI patients were assessment by echoeardiography diagram and tissue doppler ultrasound,the infarction of AMI patients was observed by ECG. The relationship of plasma BNP and these indicators of AMI patients were ana- lyzed. Results The BNP levels in AMI patients group was significantly higher than the normal population [(3784.57± 634.50)pg/mL vs (68.35±6.97)pg/mL, P = 0.000]. There was a little difference level of BNP in different infarct location AMI patients, the difference was not statistically significant (P 〉 0.05). AMI patients with left ventricular ejection frac- tion and plasma BNP level was negatively correlated (P = 0.005, r = -0.53). Killip grade Ⅲ, Ⅳ level AMI patients with plasma BNP levels significantly higher than I, class II patients, the difference was statistically significant (P〈 0.05). Conclusion The plasma BNP levels may be as an important indicator to assess left ventricular systolic function for AMI patients.
