摘要
目的探讨黄嘌呤氧化酶(xanthine oxidase,XO)在慢性心力衰竭(Chroinc heart failure,CHF)患者中与肌钙蛋白T(Troponin T,c Tn T)、B型钠尿肽(B-Type Natriuretic Peptide,BNP)的相关性。方法选择新疆维吾尔自治区人民医院2015年3月-2016年9月间符合CHF诊断标准的患者73例为病例组,根据患者左心室射血分数(Left ventricular ejection fraction,LVEF)将病例组分为3组,LVEF≤40%组29例,40%<LVEF≤50%组21例,LVEF>50%组23例。同时选择同期在我院体检中心健康体检者36例,且LVEF均正常。结果各组间基本资料的比较差异无统计学意义,P>0.05;心衰组与健康对照组XO之间差异比较二者差异有统计学意义,P<0.05;心衰组XO浓度水平(28832.32(1920,3172.5))明显高于健康对照组XO浓度水平(1223.72(683,1476));心衰组内LVEF≤40%组,40%<LVEF≤50%组,LVEF>50%三组之间XO、BNP、CTn T差异有统计学意义,P<0.05;且随着LVEF值的升高,XO、BN P、CTn T的浓度降低;结论联合监测XO、BN P、cT n T水平或许有助于慢性心力衰竭患者的风险预测及严重程度的评估,可为其治疗提供新思路。
Objective To discuss the correlation on xanthine oxidase in patients with chronic heart failure and troponin T,BNP.Methods 73 cases from April 2015 to April 2016 were diagnosed as the case group,according to the patients with left ventricular ejection fraction（LVEF）,the patients were divided into 3 groups. LVEF was less than or equal to 40%（n=29）,40%LVEF〈50%（n=21）,LVEF〉50%（n=23）. The same period of choice were not consistent with the diagnosis of chronic heart failure criteria for the diagnosis of other cardiovascular diseases in patients with 36 cases,and LVEF were normal. Results Among the groups of the data comparison,P〉0.05,the difference was not statistically significant. The difference of XO between the heart failure group and the healthy control group,P〈0.05,the difference between the two groups have statistical significance,heart failure group XO（28832.32（19203172.5）） was significantly higher than that of the healthy control group（1223.72（6831476）;heart failure group LVEF was less than or equal to 40% of the group,40%〈LVEF was less than or equal to 50% of the group,LVEF〉50% between the three groups XO,BNP and cT n T differences have statistical significance,and the XO,BNP and cT n T concentration decreased with the increase of left ventricular ejection fraction（LVEF）. Conclusion Combination of monitoring of XO,BNP and cT n T levels is helpful to improve the diagnostic value of patients with chronic heart failure,and provide a new idea for the prediction and treatment of patients with chronic heart failure.
