摘要
目的:探讨规律运动对糖尿病患者心肺耐力的影响,为糖尿病患者的科学运动做出更为准确可靠的指导。方法:查阅数据库Medline和SPORT Discus搜索相关文献,根据纳入标准和排除标准选择文献,并进行质量评定和相关数据提取。结果:1)规律运动对2型糖尿患者心肺耐力影响共有24篇文献(33项研究)满足入选标准。Meta分析发现,规律运动使2型糖尿患者VO2max值增加了16.1%(P<0.01)。2型糖尿病患者按照60%~79%VO2max的运动强度进行运动可以显著提高VO2max值(P<0.01)。有氧耐力运动、有氧耐力运动加抗阻练习均可以提高2型糖尿病患者VO2max,但前者效果更好(P<0.01)。运动干预时间超过一年比少于一年对2型糖尿病患者VO2max的提高越明显(P<0.01)。2)规律运动对1型糖尿患者心肺耐力影响有4项研究入选,规律运动后1型糖尿患者的VO2max值增高,但无统计学意义(P>0.05)。结论:规律运动可以明显改善2型糖尿病患者的心肺耐力。60%~79%VO2max运动强度可以显著提高2型糖尿病患者心肺耐力。有氧运动、有氧运动加抗阻练习均能提高2型糖尿病患者心肺耐力水平。运动干预时间越长,干预效果越明显。而规律运动对改善1型糖尿病患者心肺耐力的作用还不明确,有待于进一步的研究。
Objective: The aim of this study is to find out the effect of regular exercise training on cardiorespiratory fitness in diabetes mellitus.Medline and SPORT Discus were searched for related trials according to inclusion criteria and exclusion criteria.The trials were assessed by Jadad scales.And related data were extracted and analyzed.The result showed: 1) There are 33 studies searched about exercise intervention on cardiorespiratory fitness in type Ⅱ diabetes mellitus.Regular exercise can increase the VO2max about 16.1% in type Ⅱ diabetes mellitus(P0.01).60%~79% VO2maxintensity exercise can improve the VO2max(P0.01).Both moderate exercise and moderate plus resistance exercise can improve the VO2maxin type Ⅱ diabetes mellitus,and the former is more effective(P0.01);2) There are only 4 studies satisfied the criteria,and regular exercise can’t improve the VO2max in type Ⅰ diabetes mellitus(P0.05).The results suggest: regular exercise can improve cardiorespiratory fitness in type Ⅱ diabetes mellitus,both moderate aerobic exercise and moderate aerobic plus resistance exercise can improve cardiorespiratory fitness.60%~79%VO2maxintensity exercise can improve the cardiorespiratory fitness.It’s not sure whether regular exercise can improve cardiorespiratory fitness in type Ⅰ diabetes mellitus.
