摘要
目的通过组织细菌定量、血浆内毒素含量测定,评价中药大成汤对阻断缺血再灌注损伤后肠道细菌移位、肠源性脓毒症发生的作用。方法将24只大鼠分为正常对照组、实验对照组、大成汤治疗组,每组8只,股静脉抽血检测内毒素水平,无菌条件下取十二指肠中段、空肠中段、结肠中段、肠系膜淋巴结、肝脏、脾脏、胰腺进行组织匀浆和细菌培养、定量。结果实验对照组大鼠静脉血中内毒素水平明显高于空白对照组和大成汤组(P<0.05);十二指肠、小肠、结肠组织的细菌定量在大成汤与实验对照组比较,差异有统计学意义(P<0.05)。结论全身缺血再灌注损伤后,肠黏膜屏障功能不全;大成汤对防治内源性细菌移位、肠源性脓毒症有一定作用;细菌移位主要是通过淋巴而不是通过血液途径转移。
Objective To evaluate the preventive and therapeutic effects of Dacheng decoction on intestinal originated sepsis caused by ischemical reperfusion injury in rats, through measuring the content of bacterium in tissue and endotoxin in plasma. Methods Twenty-four rats were randomly divided into 3 groups, normal control group, experimental control group and Dacheng decoction group, with 8 rats in each group. The blood specimens were taken out from femoral vein,and the levels of endotoxin were detected. The rats' tissues were taken out including middle-dodecadaetylon, middle jejunum, middle-colon, lymphonodi mesenterici, liver, spleen and pancreas, then, which were performed tissue homogenate, bacterial culture and quantitation. Results The endotoxin levels in experimental group were significantly higher than those in normal control group and Dacheng decoction group( P 〈 0.05 ). There were significant differences in bacterial quantitation among duodenum, small intestine,colon( P 〈 0. 05 ). Conclusion Intestinal sepsis and bacterial translocation phenomenon occur after intestinal ischemia-reperfusion injury caused by hemorrhagic shock in rats. The Dacheng decoction plays a certain role in the prevention and treatment of endogenous bacterial translocation and intestinal sepsis. Bacterial translocation passes mainly lymphatic metastasis instead of blood way.
