摘要
目的探讨局麻下超声引导微通道经皮肾镜气压弹道碎石取石术(MPCNL)治疗复杂性肾结石的临床效果和安全性。方法回顾性分析解放军59医院2009年1月～2012年2月采用局麻下超声引导MPCNL治疗复杂性肾结石147例的临床资料。结果本组均顺利完成手术,手术时间95～155 min,出血量<100 ml,术中Ⅰ期结石取净84例,Ⅱ期结石取净57例,术后结石少量残留6例,结石取净率为95.9%,均无严重并发症发生。术后随访半年,均无结石复发及其他不适。结论局麻下超声引导MPCNL治疗肾脏复杂性结石具有损伤小、安全、费用低、操作简便、恢复快、疗效满意、无射线损害且结石清除率高等优点,值得临床推广。
Objective To evaluate the clinical efficacy and safety of microchannel percutaneous nephrolithotomy（MPCNL） under the supersonic guidance and local anesthesia in treatment of kidney stones.Methods Clinical data of 147 patients with kidney stones who were treated with MPCNL under the supersonic guidance and local anesthesia during January 2009 and February 2012 in our hospital were retrospectively analyzed.Results All patients were performed MPCNL successfully,the operation time ranged 95-155 minutes,and bleeding volum was less than 100 ml.There were 84 patients with complete removal at intraoperative stageⅠ,57 patients with complete removal at stage Ⅱ and 6 patients with postoperative residual stones.The removal rate was 95.9%.There was no serious complication.With follow-up for half year there was no relapse or other complication.Conclusion The method of MPCNL under the supersonic guidance and local anesthesia in treatment of kidney stones is of less trauma,safety,inexpensive cost,simple handling,efficiency,no phototoxis and high removal rate of calculi.
