摘要 目的探讨经皮肾镜碎石术肾盂冲洗的安全压力。方法建立活体猪高压肾盂冲洗模型,在不同级别压力下持续肾盂冲洗10分钟,然后分别穿刺肾实质,行HE染色及六胺银染色,在光镜下观察不同压力级别肾单位结构的变化。结果当肾盂冲洗的压力在200mmHg以下时,肾单位结构保持完整。当肾盂冲洗压力超过250mmHg,肾单位结构出现不可逆破坏。结论经皮肾镜碎石术肾盂冲洗理想压力不超过200mmHg。 Objective To explore the safe pressure limit of intrapelvic perfusion under percutaneous nephrolithotripsy(PNL).Methods Swine high-pressure intrapelvic perfusion model was established under different pressure, and renal biopsy specimens were taken after 10-minute intrapelvic perfusion. All specimens were observed under light microscope through HE and hexamine silver staining. Result The higher the intrapelvic pressure, the more obvious change of nephron structure. When the pressure was 200mmHg below, the nephron structure remained complete. When it went up to 250mmHg or above, the nephron structure was damaged irreversiblely. Conclusion The ideal intrapelvic perfusion for PNL was lower than 200mmHg.
