摘要
目的探讨急、慢性肾损伤患者血清和尿液中中性粒细胞明胶酶相关脂质运载蛋白(NGAL)的变化及其临床价值,并与血肌酐(Scr)浓度、肾小球滤过率(GRF)做比较。方法选择急性肾损伤(AKI)患者95例,原发性慢性肾脏疾病(CKD)患者40例,用ELISA法检测血清和尿液中NGAL的浓度,并与50例健康体检正常者作比较,分析血清和尿液中NGAL的变化以及与肾小球滤过率(GFR)之间的相关性。根据AKI临床分期标准,将AKI组分为3期,观察各组血清和尿液中NGAL的变化。结果 AKI组、CKD组和健康对照组血清中NGAL水平分别为:678.6±45.3μg/L、374.5±63.1μg/L、49.6±8.3μg/L,尿液中分别为:75.5±10.2μg/L、44.3±9.8μg/L、7.45±0.66μg/L。AKI、CKD组血清NGAL、尿液NGAL与健康对照组相比,差异均具有统计学意义(P<0.05);AKI分期越高,血、尿NGAL水平越高,血清NGAL和尿液NGAL水平与GFR存在负相关(相关系数分别为:血清NGAL r值为-0.867,P<0.05;尿液NGALr值为-0.756,P<0.05)。结论 NGAL可作为急性肾损伤的指标,在重症患者和慢性肾脏病并发AKI时,早期、连续检测血清或尿液NGAL水平,对于及时、准确地了解肾功能损伤的进展,及早制定相应的干预措施,具有十分重要的意义。
Objective To investigate the clinical change and significance of.neutrophil gelatinase-associated lipocalin (NGAL) in patients with acute and chronic renal damage and the comparison with serum creatinine (Scr) and glomerular filtration rate(GRF). Methods the NGAL concentration in serum and urine were detected by ELISA in 95 Acute kidney injury (AKI) patients and 40 primary chronic kidney disease (CKD) patients, 50 healthy subjects as control group. The changes o{ NGAL in serum and urine and relationship to glomerular filtration rate (GFR) were analyzed. The serum and urine NGAL AKI in each group divided by clinical staging criteria were also observed. Results NGAL level in the serum of the AKI group,CKD group and healthy control group were:678.6±45.3μg/L,374.5±63.1μg/ L,49.6±8.3 μg/L;75.5±10.2 μg/L,44.3±9.8 μg/L,7.45±0.66μg/L in urine. The NGAL in serum and urine in AKI and CKD group serum were higher than in control, the differences were statistically significant(P〈0.05) ;NGAL levels in serum and urine increased accompany with AKI phase,and NGAL levis were negatively correlated with GFR (in serum:r=0 867,P〈0.05 ;in urine:r=-0. 756,P%0.05). Conclusion MGAL can be used as indicators of acute kidney injury,and the early and continuous detection of serum or urine NGAL level in critically ill patients and chronic kidney disease complicated with AKI,have great significance in the understanding of the progress of renal damage and the early development of appropriate intervention measures.
