摘要 目的评价3.0T磁共振在胰腺神经内分泌肿瘤诊断中的应用价值。方法回顾性分析6例经手术病理证实的胰腺内分泌肿瘤的患者,比较T1WI,T2WI,DWI和动态增强四个序列对病灶的显示情况。结果 6例患者中,其中1例有5个病灶,共10例病灶。病灶最大径平均值为2.12cm,在T1WI中3例低信号,5例稍低信号,2例等信号。在T2WI中6例高信号,2例稍高信号,2例信号不均匀,表现为等/高混杂信号;DWI中,去除囊变坏死等部分,10例均为高信号。动态增强方式有以下几种:6例病灶整个动态扫描周围表现为均匀轻中度强化,2例病灶动脉期中度或明显均匀强化,门脉和延迟期有所下降但仍高于周围正常胰腺实质信号,2例病灶中央明显囊变坏死,囊变坏死区无强化,周围呈持续性环状强化。结论多数胰岛细胞瘤TIWI上表现为等低信号,T2WI上表现为高信号,DWI上均表现为高信号,动态增强上MRI动脉期明显强化。 Objective In order to assess the application value of 3.0 T MRI in the diagnosis of pancreatic neuroendocrine tumor. Methods 6 cases were retrospectively analyzed by surgery pathology confirmed pancreatic endocrine tumor patients,average age,52 years comparative T^WI,T2WI,DWI and dynamic enhanced four sequence on the lesions of display case. Results In 6 cases,including 1 case with five lesions,10 cases of focal lesions maximum diameter mean value was 2+12 cm,3 cases in T1WI of low signal,5 cases of slightly low signal,2 cases,etc in T2WI signal in 6 cases,2 cases of high signal higher signal,2 cases of signal was not uniform,for the performance of the/high hybrid signal. DWI,remove capsule variable necrosis part,10 cases were high signal dynamic enhanced mode has the following kinds:6 cases of focal the whole dynamic scanning around for the performance of the uniform mild-to-moderate reinforcement,2 cases of arterial lesions moderate or obvious uniform strengthening,portal and delay declined but still higher than the surrounding normal pancreas essence signal,2 cases of focal central obvious sac change necrosis,capsule become necrotic area around without reinforcement,a persistent annular reinforcement. Conclusion Most of the islet cell tumor T1WI performance for the low signal,etc,for the performance of the high signal on T2WI,DWI all performance for high signal,dynamic enhanced MRI on arterial obvious reinforcement.
