摘要
目的:探讨血管细胞黏附分子-1(VCAM-1)和肿瘤坏死因子-α(TNF-α)在重度子痫前期合并胎儿生长受限(FGR)时孕妇血浆及胎儿脐血中表达的水平及在发病机制的作用。方法:用酶联免疫吸附法(ELISA)定量检测28例重度子痫前期合并FGR患者(SPF组)、30例单纯重度子痫前期患者(SP组)及30例正常妊娠晚期妇女(NP组)血浆及脐血中VCAM-1和TNF-α的含量。结果:SPF组与SP组患者血浆中VCAM-1和TNF-α的水平均明显高于NP组(均P<0.05),且两者在SPF组又明显高于SP组(均P<0.05)。脐血中VCAM-1的表达在各组间比较差异无统计学意义(P>0.05)。SPF组与SP组脐血中TNF-α水平明显高于对照组(均P<0.05),但SPF组与SP组比较差异无统计学意义(P>0.05)。母血中SPF组和SP组VCAM-1与TNF-α的表达水平均呈正相关(r=0.88,P<0.01;r=0.75,P<0.05)。结论:母血中VCAM-1和TNF-α的高表达与重度子痫前期密切相关,合并FGR患者中存在更为严重的内皮细胞损伤。
Objective: To explore the role of VCAM-1 and TNF-a in the pathogenesis of severe preeclampsia with fetal growth restriction (FGR). Methods: A cross-sectional study was conducted with 88 women in the following categories. (1) severe preeclampsia merger FGR patients (SPF, n = 28), (2) pure severe pre-eclampsia patients (SP, n =30), and (3) normal pregnancy advanced women (NP, n ----30). The ex- pression of VCAM-1 and TNF-a in plasma and umbilical cord was examined by ELISA. Results. The expression of VCAM-1 and TNF-a in the SPF and SP groups were significantly higher than that in the NP group ( P〈0.05), and the expression of VCAM-1 and TNF-a in the SPF group was higher than that in the SP group ( P〈0.05). There were no significant differences in cord blood VCAM-1 among the three groups ( P〈0.05). The expression of cord blood TNF-a was significantly higher in the SPF and SP than in the NP groups (P d0.05), however, there was no significant difference between the SPF and SP groups ( P〈0.05). VCAM-1 in maternal blood was positively correlated with TNF-a in the SPF and SP groups ( r = 0.88, P〈0.01 ; r = 0.75, P〈0.05; respectively). Conclusion. The high expression of VCAM-1 and TNF-a in maternal blood and severe preeclampsia closely relates to the presence of more severe endothelial cell injury, combined FGR patients.
