摘要
目的了解广州市艾滋病病例综合管理服务体系的现状及其利用状况。方法采取定性调查和定量调查相结合的方法,分析服务体系的现状和服务对象对体系的利用情况。结果广州市艾滋病病例综合管理服务体系由医疗机构、疾控机构、社区卫生服务机构与社会组织共同组成,可以提供多层次、有序和规范的服务。艾滋病病例对疾控机构和抗病毒治疗机构的接受率及高满意度的比例在70.3%～93.5%之间;对社区卫生服务机构的服务接受率和高满意度的比例分别为71.2%和50.0%;对结核筛查治疗机构、美沙酮维持治疗服务点的知晓率、服务接受率和高满意度的比例低于50%;对阳性结果首次告知服务接受率和高满意度的比例分别为92.3%和55.6%;对关怀支持服务的知晓率和服务接受率<50%,而高满意度的比例>60%。结论广州市艾滋病病例管理服务体系已经发挥了巨大作用,但服务平台及服务内容尚未得到充分利用;社区卫生服务机构、首次阳性结果告知、结核筛查、吸毒行为干预和社会关怀支持等服务亟待加强。
Objective To realize the current status of the Guangzhou service system of HIV/AIDS comprehen- sive case management and its application. Methods The current status of the service system and its application were analyzed by combined qualitative and quantitative surveys. Results The Guangzhou service sy,;tem of HIV/ AIDS comprehensive case management was composed of medical institutions, CDCs, community health service institutions and NGOs, which provided comprehensive, well-organized and standardized services. The rates of accept- ance and satisfaction for CDCs and ART institutions were between 70.3% and 93.5% respectively, while the rates of acceptance and satisfaction for community health services institutions were 71.2 % and 50.0 %, respectively. The rates of awareness, acceptance and satisfaction for tuberculosis screening institutions and methadone maintenance treatment clinics were lower than 50 %. The rates of acceptance and satisfaction for the first-time notification of HIV positive results were 92.3% and 55.6%, respectively. The rates of awareness and acceptance of the service of care and support were generally lower than 50%, however, the rate of satisfaction was more than 60%. Conclusion The Guangzhou service system of HIV/AIDS comprehensive case management plays an important role, but the sys- tem platform and all kinds of services have not been fully used. Community health services, first-time notification of HIV positive results, tuberculosis screening, interventions for drug users and the services of care and support should be strengthened.
