Tracking detection of serial serum in early HIV-RNA positive cases by different methods

摘要 目的:比较不同方法对早期HIV感染者血清检测的"窗口期"及其敏感性,为早期诊断和预防提供依据。方法:用多种方法对HIV核酸检测阳性的早期感染者进行追踪检测,观察抗原抗体、病毒载量的动态变化及早期临床症状。结果:梅里埃4代ELISA检测的"窗口期"为三周。国产4代、3代ELISA及快检试剂的"窗口期"为四周。首次报告早期感染者血清存在HIV抗体下降的"波谷期"。相应症状出现于暴露23 d时;gp160条带检出于暴露30 d;病毒载量于暴露80 d时达高峰。结论:在HIV感染早期,要选择敏感性高的检测试剂,以防漏检。 Objective :To compare the window period and sensitivity of the different methods in detection of early HIV infection, and provide the basis of early diagnosis and prevention of HIV infection. Methods:Multiple methods were used to observe the antigens, antibodies, dynamic changes of viral load and early clinical symptoms in early HIV - RNA positive cases. Results : The window period of the fourth generation ELISA of Marcel Merieux was three weeks. While that of the domestic fourth and the third generation ELISA and other quick test reagents was four weeks. First reported early HIV infections had a "trough period" of serum antibody. The relational clinical symp- toms appeared on 23 days after exposure. The gpl60 band was found on the 30 days. While on 80 days, the viral load came to the peak. Conclusion: In early detection of HIV infection, it is important to choose the high - sensitive test reagents, avoiding leaked cases.
