摘要
目的了解一组泛耐药鲍氏不动杆菌中β-内酰胺类药物耐药机制。方法收集南通大学附属医院2011年1-4月临床标本中分离的泛耐药鲍氏不动杆菌共20株,采用聚合酶链反应(PCR)及序列分析的方法分析A、B、C、D 4类共33种β-内酰胺酶编码基因、膜孔蛋白carO编码基因、插入序列ISaba1与β-内酰胺酶OXA-23基因(ISaba1-OXA-23)连锁检测。结果 20株鲍氏不动杆菌均检出TEM-1、ADC-30、OXA-23等3种β-内酰胺酶基因,检出率均为100.0%,carO基因均存在有义突变,插入序列ISaba1与β-内酰胺酶OXA-23基因(ISaba1-OXA-23)连锁检测均为阳性。结论该组鲍氏不动杆菌耐β-内酰胺类药物与产TEM-1、ADC-30、OXA-23等3种β-内酰胺酶、膜孔蛋白CarO编码基因存在有义突变、插入序列ISaba1与β-内酰胺酶OXA-23基因(ISaba1-OXA-23)连锁检测阳性相关。
OBJECTIVE To investigate the distribution and variety of β-lactamace resistance genes in a group of pan-drug resistant of Acinetobacter baumannii(A.baumannii) strains.METHODS From Jan 2011 to Apr 2011,a total of 20 strains of pandrug-resistant A.baumannii were collected from Affiliated Hospital of Nantong University,China.Then,33 kinds of beta-lactamases(Class A,B,C,D),outer membrane protein gene carO and chain detection of insertion sequence ISaba1 and beta-lactamase OXA23(ISaba1-OXA23) were analyzed by PCR and verified by DNA sequencing.RESULTS In 20 strains of A.baumannii,3 kinds of beta-lactamases,TEM-1,ADC-30,OXA-23,were all positive,and sense mutations existed in all carO gene,and linkage detection of the inserted sequence ISaba1 and beta-lactamase OXA23(ISaba1-OXA23) was positive.CONCLUSION The 3 kinds of beta-lactamases including TEM-1,ADC-30,and OXA-23,sense mutations in carO gene,the insertion ISaba1,and the positive chain detection of beta-lactamase OXA23(ISaba1-OXA-23) play a key role in the resistance to beta-lactams in this group of A.baumannii strains.
