摘要
目的调查泛耐药鲍氏不动杆菌获得性耐药相关基因和可移动遗传元件携带状况,并分析两者的相关性。方法收集医院2008年1-12月住院患者送检痰液标本中分离的泛耐药鲍氏不动杆菌共20株,采用聚合酶链反应的方法分析54种β-内酰胺类、喹诺酮类、氨基糖苷类获得性耐药相关基因及12种可移动遗传元件遗传标记,并对检测结果作指标聚类分析。结果 20株泛耐药鲍氏不动杆菌共检出获得性β-内酰胺类耐药基因TEM-1、ADC-3和OXA-30,三者阳性率均为100.0%;检出的5种获得性氨基糖苷类耐药基因中,aac(6′)-Ⅰb、ant(3″)-Ⅰ和aph(3′)-Ⅰ基因阳性率为100.0%,aac(3)-Ⅰ基因阳性率为75.0%,armA基因为5.0%;外排泵基因qacE△1、adeB和可移动遗传元件遗传标记基因intⅠ1、tnp513、tnpU、ISaba1、IS26的阳性率均为100.0%;其余51种基因未检出。结论从该组泛耐药鲍氏不动杆菌指标聚类分析中可见,β-内酰胺酶基因TEM-1、ADC-30、OXA-23,氨基糖苷类修饰酶基因aac(6′)-Ⅰb、ant(3″)-Ⅰ,抗菌制剂外排泵基因qacE△1和abeB与可移动遗传元件intⅠ1、tnpU、tnp513、IS26、ISaba1基因遗传标记高度相关。
OBJECTIVE To investigate the distribution of acquired resistance genes and mobile genetic elements in pandrug-resistant Acinetobacter baumannii,as well as the correlation between them.METHODS From Jan 2008 to Dec 2008,totally 20 strains of pandrug-resistant A.baumannii isolated from inpatients were collected from a hospital in China.Then,54 kinds of acquired resistance genes to beta-lactams,quinolones,aminoglycosides,and 12 kinds of genetic markers of mobile genetic elements were analyzed by PCR.Furthermore,the index cluster analysis was performed.RESULTS In 20 strains of pandrug-resistant A.baumannii,3 kinds of acquired beta-lactam-resistance genes were detected,including TEM-1(100.0%),ADC-3(100.0%),and OXA-30(100.0%),and in 5 kinds of acquired aminoglycoside-resistance genes,there were aac(6′)-Ⅰb,ant(3″)-Ⅰ and aph(3′)-Ⅰ detected with the positive rates of 100.0%,the positive rate of aac(3)-Ⅰ was 75.0%,armA 5.0%;the positive rates of efflux pump genes qacE△1 and adeB and the genetic markers of mobile genetic elements intⅠ1,tnp513,tnpU,ISaba1,and IS26 were 100.0%;the rest of 51 kinds of genes couldn′t be detected.CONCLUSION In this group of pandrug-resistant A.baumannii,index clustering analysis shows that the beta-lactamase genes TEM-1,ADC-30,OXA-23,aminoglycoside modifying enzyme genes ant(3″)-Ⅰ,aac(6′)-Ⅰb,and antibacterial agents efflux pump gene qacE△1,abeB are highly correlated with the genetic markers of mobile genetic elements intⅠ1,tnpU,tnp513,IS26,and ISaba1.
